0005583183 2016-09-28 Final Restoration No Violations On the day of my inspection I witnessed Chipco LLC remove the flowline and repair grade completing the restoration. The site passed final restoration according to ORC 1509.072B.
0482987270 2015-10-29 Plug / Plug Back No Violations On the day of my inspection I witnessed the tubing had no pressure and the valves on the wellhead were inoperable. Energy Services had to perform a hot tap to check the pressure on the casing. After successful hot tap monitors detected the well had H2S readings of 20ppm. Sweeny Inc. pumped 20 gallons of sulfur clear followed by 80bbls of water down the backside. Chipco LLC pulled on the tubing discovering the well had a packer installed. Chipco was able to free up packer and lay down 7 joints of 1.5 inch tubing before getting stuck. Chipco tripped the 7 joints of 1.5 inch tubing back in forcing to bottom. Decision was made to shoot tubing off and set bottom plug and then perforate the Big Lime on Monday.
0616438908 2016-05-05 Plug / Plug Back No Violations On the day of my inspection I witnessed the restoration issues have not been addressed. I placed call to Chipco and spoke with Chip Smith and he stated as soon as the weather breaks the issues will be addressed.
0662274434 2015-11-03 Plug / Plug Back No Violations On the day of my inspection I witnessed Chipco LLC tag Big Lime plug at 3645 feet. Chipco ripped 4.5 inch casing at 3465 feet. ODNR Inspector Austin Gutridge will complete the plugging operations.
1446468459 2015-11-02 Plug / Plug Back No Violations On the day of my inspection I witnessed H2S readings were at 0ppm. Appalachian Well Surveys was onsite and tripped in the well with an 1.5 inch tubing cutter sat down at 5730 feet. AWS cut tubing off at 5700 feet and Chipco pumped a 30 sack 400 foot Class A bottom plug from 5700 feet to 5400 feet. Cement was displaced with 13.3bbls of water. Chipco pulled 1.5 inch tubing to 4000 feet and AWS perforated the Big Lime with 10shots from 4124 feet to 4114 feet. Chipco tripped in 3 joints of tubing gained circulation and pumped a 60 sack 200 foot Big Lime plug from 4124 feet to 3924 feet. Cement was displaced with 9.9bbls of water. Chipco pulled remaining 1.5 inch tubing and will rip 4.5 inch casing.
1446647805 2015-11-03 Plug / Plug Back No Violations The day of my inspection Chipco was on location with their service rig, pump truck, and cement truck. I witnessed Chipco set the rip plug from 3422 ft. to 3272 ft. they pulled the pulled the 4.5 inch up to 1560 ft. They set a 200 ft. plug at the bottom of the 8.625 inch surface casing 1560 ft. to 1360 ft. Chipco pulled up the 4.5 inch to 200 ft. and set the 200 ft. surface plug from 200 ft. to surface. All plugs were set with Class A cement through 4.5 inch casing. Surface plug stayed at surface.
1459196820 2016-03-28 Plug / Plug Back No Violations On the day of my inspection I met with Chip Smith of Chipco LLC to discuss the deficiencies of the reclamation. The location needed to be redressed, seeded, mulched and the flowline removed. Mr. Smith stated when it dries out he will have a crew address the issues.
1592110851 2015-12-17 Plug / Plug Back No Violations On the day of my inspection checking the status of reclamation, I witnessed the site has been seed and mulched. The location was rutted up with the flowline still present. I placed call to Enervest stating my findings and that it would have to be corrected in order to pass final restoration.
