0082025024 2014-12-31 Plug / Plug Back No Violations On the day of my inspection I witnessed that Chipco LLC had removed 4000 feet of 4.5 inch casing and ran 3945 feet of 2.375 inch tubing back in the well bore. Petroset was on site rigged up, gained circulation and pumped a 115 foot 33 sack Class A cement partial Big Lime plug from 3945 feet to 3830 feet weighing 15.6ppg with a yield of 1.18cuft/sk. Chipco LLC pulled 4 joints of 2.375 inch tubing and Petroset reverse circulated until the well bore was clear. Petroset conditioned the well bore with gel. The remaining Big Lime plug will be set when the new 4.5 inch production casing is landed and cemented on Friday completing the plug back.
0126905177 2014-12-29 Plug / Plug Back No Violations On the day of my inspection I was on site to witness the big lime plug on plugging back of said well. Chipco LLC had run their ripper down to 4500 feet and was unable to get telegraph. Chipco unsuccessfully tried to rip at 4400 feet, 4300 feet, 4200 feet, 4130 feet and 3900 feet. Enervest decided to have Appalachian Well Service shoot casing off tomorrow.
0247894848 2013-10-28 Production Wells Violations Noted On the day of my follow up inspection on the compliance notice agreement, I witnessed at the well site the containment dike had been repaired, there was containmented soil inside the containment dike and around the production tank. The ball valve under the concoil catcher at the well head was open, the valve on the production line leading from the concoil catcher to the separator was closed, there was no date or production on the chart, I reviewed the Division of Oil and Gas production records for the year of 2012 and found they do not meet The Division of Oil and Gas Standards, I placed a phone call to Berry Enterprise to inform them of the containmented soil left inside the dike, I also informed Berry Enterprise to remove the containmented soil and take it to an approved land fill, the soil removed was to be replaced with good soil. I asked Berry Enterprise why the well was not in production and was told by Mr. Berry on the date of 10/03/2013 the well had down hole problems and was going to schedule a service rig to make the repairs, on the date of 10/28/2013 there was no change in the status of the well. The well is in violation of 1509-062H and 1509-12B, requirements to meet The Division of Oil and Gas Standards are to make the well capable of commercial production with fourteen day or plugging is required within thirty days. Failure to comply may result in Forfeiture of bond.
0666649239 2014-12-24 Plug / Plug Back No Violations On the day of my inspection I witnessed that Chipco LLC had retrieved the packer and ran tubing back in and set a 400 foot 30 sack Class A cement Clinton plug from 5316 feet to 4916 feet weighing 15.6ppg with a yield of 1.18cuft/sk. Chipco will lay tubing down and rip casing on Friday with plans to complete plug back operations next week.

1371484907 2013-06-17 Production Wells No Violations Pryor to my inspection, I received a phone call from David Brook who owns the land that the well is on, his concern was of production and why the chart recorder did not have any record for the last year. I placed a phone call to Berry Enterprise and was told the well had a problem down hole for one year and was being used for house gas. Berry Enterprise just recently had a service rig on the well for repairs and is currently back on the sales line. I requested a copy of there production records from the year of 2010/ 2011/ 2012. My inspection revealed the well is a rabbit well, all the valves were open, the pressure gauge showed one hundred and fifty pounds of pressure at the well head and one hundred and seventy pounds at the compressor, all the valves were open at the compressor, there is a separator and (1) two hundred ten barrel production tank setting inside a containment dike, the I.D. is posted on the tank, there is also a chart record at the check valve on the main line that was current at the time of my inspection.
1377103465 2013-08-21 Production Wells Violations Noted Prior to my inspection I received a phone call from David Crock with a concern of the well that is on his mother in-law property, his concern is that nothing has been done to the well since my last inspection which was on 06/17/2013, on that date I made a phone call to Berry Enterprise to inform them of the issues on the well location and of the production. On this day 08/21/2013 I witnessed at the well head the 4.5 ball valve was in the open position, the valve on the production line leading from the well over to the separator was in the closed position, the valves on the production line leading from the separator to the two hundred ten barrel production tank was in the open position, there is a small compressor setting outside the containment dike, the valves on the compressor was in the open position, there was a power box with a lead going to the compressor, the fuses in the power box was in the off position, the containment dike around the compressor and the production tank is incapable of holding the contents of the tank due to improper construction and erosion, there is contaminated soil inside the dike. The production reported for the year of 2012 does not meet The Division of Oil and Gas Standards. The well is in violation of / 9-9-02 General Safety / 9-1-07 Well operations causing pollution and contamination / 9-9-05 No SPCC dike or failure to keep dike free of water or oil / Dike not able to prevent escape of brine or other wastes / Failure to keep dike free of brine or other wastes / Temporary inactive, failure to notify within 60 days of commenced production / Well shall be plugged. The requirements to meet The Division of Oil and Gas Standards will be to make the well capable of commercial production within 30 days; or plugging is required within 30 days. Failure to comply may result in Forfeiture of Bond.
1420237127 2015-01-02 Plug / Plug Back No Violations On the day of my inspection I witnessed that Chipco LLC had ran 3812 feet of 4.5 inch 10.5#/ft casing and Universal Well Service was on site gained circulation, pumped 15bbls of gel, 5bbls of fresh water and cemented casing with 350sack of 50/50 Poz cement weighing 14.4ppg with a yield of 1.29cuft/sk. Cement was displaced with 60.4bbls of fresh water completing the plug back. Latch down plug was landed at 585psi and pressured up to 1085psi for 5 minutes and the floats held.

1920947145 2014-12-30 Plug / Plug Back No Violations On the day of my inspection I witnessed that Appalachian Well Service was on site running a Gamma Ray-CCL-VDL Cement Bond log and found a free point at 4000 feet. Appalachian Well Service went back in the hole and shot the casing with a 3.625 inch casing cutter at 4000 feet. Chipco LLC freed up casing and pulled 1 joint out. Enervest wants to try to circulate gel around the backside to clean out hole prior to pulling casing, running tubing and setting the lime plug. There was no available service company to pump gel until tomorrow.
