0014202061 2016-03-10 Production Wells No Violations On the day of my inspection I witnessed the production equipment included a 210bbl tank, a horizontal separator, a motor valve, a drip, a 2 inch sales meter with a .75 inch orifice and a chart recorder with a dated chart all within an earthen dike. The well was equipped with 8.625 inch casing, 4.5 inch casing, 1.5 inch tubing, a plunger lift system and domestic gas service with all valves in the closed position. Identification was present and production records were current. A plugging permit was issued on 3/7/16 and expiring on 3/7/18 for the said well.
0116681109 2017-05-08 Production Wells No Violations On the day of my inspection I witnessed Chipco LLC Rig #6 was onsite and rigged up with no Hydrogen Sulfide (H2S) being detected but a heavy odor was present. Monitors were in place along with a ventilation fan. B & E Well Treating arrived and pumped 200 gallons of Sulfa-clear down the 1.5 inch diameter tubing and pumped 400 gallons of Sulfa-clear down the 4.5 inch diameter casing. Chipco will commence plugging operations tomorrow.
0202534194 2017-05-09 Plug / Plug Back No Violations On the day of my inspection utilizing an ODNR Honeywell PhD6 meter revealed a LEL of 0 and H2S reading of 0 parts per million (ppm). Also utilizing ODNR Tango H2S detector revealed 0 ppm. Chipco LLC Rig #5 spear the1.5 inch diameter tubing, verify the tubing was free and replace the top joint. Chipco pumped a 30 sack 400 foot Class A Clinton plug from 5269 feet to 4869 feet. Cement was displaced with 12 barrels of fresh water. Chipco tripped the 1.5 inch diameter tubing from wellbore and ripped the 4.5 inch diameter casing at 3800 feet and pulled 1 joint to verify the casing was free. Chipco pumped a 60 sack 200 foot Class A Big Lime cement plug from 3738 feet to 3538 feet and displaced cement with 57 barrels of fresh water. Chipco tripped 4.5 inch diameter casing to 1369 feet and will resume plugging tomorrow.

2144869835 2017-05-10 Plug / Plug Back No Violations On the day of my inspection accompanied by ODNR Inspector Dylan Pendleton, I witnessed Chipco gain circulation and pump a 60 sack 200 foot Class A bottom of the surface cement plug from 1369 feet to 1169 feet and displaced cement with 18.4 barrels of fresh water. Chipco tripped 4.5 inch casing from wellbore. Chipco pumped a 60 sack 150 foot Class A cement plug from 150 foot to surface circulating cement to surface. Instructed operator to cut casing off 30 inches below grade and install metal plate identifying well and reclaim according to ORC 1509.072B.
