0394968570 2016-05-16 Plug / Plug Back No Violations On the day of my inspection following up on Compliance Notice-1645786637 and Complaint ID 5314, H2S readings were at 70ppm on the 4.5 inch casing with a safety meeting being held with Enervest Superintendent Tim McConaha, Appalachian Well Surveys, ODNR Inspector Joe Zollars and Chipco LLC employees. AWS tripped inside the 1.5 inch tubing and shot the tubing off at 5746 feet and Chipco pumped a 30 sack 400 foot Class A Clinton cement plug from 5746 feet to 5346 feet and displaced cement with 13.4bbls of brine. Tek Solve arrived on location and setup H2S monitoring and left the site. AWS tripped inside the tubing to 17 feet above the packer and shot the tubing at 4860 feet as they were tripping out of the tubing AWS lost their tools which consisted of a collar locater and 2 sinker bars. Attempts to retrieve and drive to bottom unsuccessful decision was made to shoot tubing again at 4092 feet. Chipco pulled 2 joints of 1.5 inch tubing as AWS tripped back in to perforate the Big Lime but could not drop out of the bottom of the tubing. AWS perforated the tubing with 10 shots from 4034 feet to 4024 feet. AWS tripped out of the tubing and Chipco pumped a 30 sack 500 foot Class A Big Lime plug from 4034 feet to 3534 feet and displaced cement with 9.5bbls of brine and shutting well in. AWS will tag Big Lime plug and re-perforating tomorrow with Chipco setting an additional isolation plug.

AWS tripped inside the 1.5 inch tubing, sat down at 3280 feet and shot the tubing off at 3275 feet. AWS perforated the 4.5 inch casing with 10 shots from 3215 feet to 3205 feet. Chipco pumped a 60 sack 200 foot Isolation plug in the Ohio Shale from 3215 feet to 3015 feet and displaced cement with 7.5bbls of fresh water. After displacement H2S alarms went off at 100ppm and the well was shut-in for the day with plans to complete tomorrow.

0587024953 2016-05-13 Production Wells No Violations On the day of my inspection following up on Compliance Notice-1645786637 and Complaint ID 5314, a safety meeting was held with Enervest Superintendent Tim McConaha and Chipco LLC employees. I witnessed that a second hot tap was performed yesterday on the 4.5 inch casing with the tubing annulus receiving 600 gallons of sulfa-clear. Pressure on the annulus was at 475psi with no H2S present Chipco began treating the well with an additional 10bbls of sulfa-clear and 10ppg brine. Chipco was unable to pump on the annulus so a controlled blowdown was done resulting in spikes of H2S. Chipco pumped the additional 10bbls of sulfa-clear along with 26bbls of 10ppg brine resulting in the well becoming static with no H2S present. Chipco changed out all wellhead valves and tripped inside the 1.5 inch tubing to 5750 feet, tripping out and loading the tubing with 10ppg brine. Plugging commencement will begin on Monday 5/16/16.

1051078278 2016-05-12 Production Wells No Violations On the day of my inspection following up on Compliance Notice-1645786637 and Complaint ID 5314 accompanied by ODNR Inspector Joe Zollars, I met with Enervest Services Superintendent, Welder Jim Green and Chipco representatives and a safety meeting was held prior to hot tapping the well. H2S was detected with a fan blowing Jim Green successfully tapped the 4.5 inch casing valve nipple. A 1 inch valve was installed with H2S present and pressures of 475psi on the casing. Appalachian Well Service will be onsite later today to treat the well with Sulfa Clear and Chipco Rig #6 to be rigged up after well is treated. Tech Solv will be onsite Monday at 7:00am for H2S monitoring prior to plugging commencement and throughout with road closure from 9:00am to 2:00pm.

1645786637 2016-04-26 Production Wells Violations Noted On the day of my inspection accompanied by ODNR Inspector Joe Zollars following up on Complaint ID 5311, I witnessed that the H2S was present in the air at the entry of the access road along with a sign stating Danger H2S. Flags were placed to determine wind direction as Inspector Zollars and I approached the well from the west. I found the well equipped with 8.625 inch casing; 4.5 inch casing, 1.5 inch tubing and a plunger lift system with valves open on the tubing and valves closed on the casing with deterioration present on the casing, valves and flowline. The production equipment included a 210bbl tank, a horizontal separator, a logically controlled motor valve, a 2 inch sales meter with a .875 inch orifice and a chart recorder all within an earthen dike. Identification was present and production records were current. I placed call to Enervest and spoke with Tim McConaha and stated my findings. Mr. McConaha stated that he would treat the well with sulfur clear and come up with a plan of action to address the well. The well owner/operator shall submit a written plugging schedule to Doug Bennett at 5880 Memory Road Zanesville, Ohio 43701-9553 within 15 days, per OAC 1501:9-11-05(B), and plug the well within 30 days. The well owner/operator will be notified if the plugging schedule is approved or denied.

Chipco inspected the wellhead and checked pressure on the casing revealing 25psi and then performed a controlled release to the half round spiking the H2S readings to >100ppm. Chipco pumped 22bbls of water clearing the perforations at 1332 feet and reducing the H2S levels to 0ppm. Volume was calculated at 160bbls to the fill the annulus along with 4.5 inch casing and a total of 70bbls being pumped since the last plug set the job was shut down waiting on water. ODNR Southeast Regional Supervisor Brent Bear and I made a sweep of the surrounding shale pads and conventional sites revealing normal levels of H2S with a strong odor on London Road. Upon arrival of the water Chipco pumped an additional 45bbls which filled the annulus and the casing. AWS perforate the 4.5 inch casing at 500 feet which is 58 feet below the cement that was pumped down the annulus of the casing in 1984. AWS tripped out of the hole and the well released trapped gas from the annulus spiking the H2S readings at the half round >100ppm. Levels returned to normal with Chipco tripping in 2.375 inch tubing and a packer to a depth of 1134 feet and then circulating 45bbls of water to the half round. Chipco pumped a 60 sack 200 foot Class A bottom of the surface casing plug through the perforations from 1334 feet to 1134 feet and the cement was displaced with 4bbls of water. Chipco then released the packer and tripped out of the well to 750 feet and circulating 15bbls of fresh water. Chipco tripped out of the hole revealing that the packer along with 236 feet of 2.375 inch tubing was lost in the well. Chipco shut-in the 4.5 inch casing with completion of plugging planned for tomorrow
1996279410 2016-05-20 Plug / Plug Back No Violations On the day of my inspection following up on Compliance Notice-1645786637 and Complaint ID 5314, pressure readings were at 0psi and H2S readings were at 0ppm on the 4.5 inch casing. A safety meeting was held with Enervest Drilling Supervisor Joe Wade and Chipco LLC employees. Chipco rigged up their 4 inch tools, tripped in the hole and tagged the tubing that was lost at 905 feet. Chipco tripped out of the hole and tripped 2.375 inch tubing back in to a depth of 750 feet. Chipco pumped a 57 sack 750 foot Class A cement plug inside the 4.5 inch casing from 750 feet to surface circulating cement to surface. Chipco tripped out of the hole with the 2.375 inch tubing, cement dropped to 100 feet and was topped off to surface with an additional 8 sacks of cement. The casing will be left open until Monday for monitoring with Compliance Notice-1645786637 and Complaint ID 5314 now resolved.
