1139624213 2018-06-22 Production Wells No Violations The inspection was conducted under raining conditions. The rig had not moved in due to swampy conditions. The owner of the service rig had buried pick-up. The plug job would be postponed till better weather. The well was equipped with 8 5/8-inch casing reduced to 2 inches. There was no production equipment associated with the well. I called Chip Smith and he would move in at later date.

1484747832 2017-01-18 Production Wells No Violations On the day of my inspection I witnessed the well was equipped with 8.625 inch diameter casing reduced to 2 inches with all valves in the closed position. No production equipment could be located. No identification was present and production records were current with zero production reported since 2002. I contacted Enervest requesting their plans for the well.
1533146874 2018-08-01 Final Restoration No Violations The purpose of my inspection was a status check on a final restoration. During my inspection all debris from plugging operations have been removed and all production equipment has been removed. The lease road and location had vegetation established. Location meets final restoration requirements.
1846738544 2018-07-16 Plug / Plug Back No Violations The well consisted of 8.625 inch diameter surface casing set at 1247 feet. Chipco was on location with a derrick service rig, cement bulk truck, and cement pump truck. The bottom hole was tagged at 2225 feet. Chipco tripped 2.375 inch diameter tubing into 2192 feet. Chipco pumped a 500 foot bottom hole plug a crossed the Ohio Shale with 152 sacks of class A cement. The plug was displaced with 4.2 barrels of fresh water. Chipco tripped the 1.5 inch diameter tubing up to 1374 feet. A bottom of surface casing plug was pumped from 1374 feet to 1174 feet. The plug was displaced with 2.9 barrels of fresh water. Chipco tripped up to 150 feet and pumped a surface plug with 45 sacks of class A cement. The cement circulated to surface, the tubing was tripped from hole, and the 8.625 inch diameter casing was topped off with class A cement. The cement stayed at surface.
