0410393558 2015-02-03 Production Wells No Violations On the day of my inspection following up on NOV- 687342636, I witnessed that the production equipment included a 210bbl tank, a horizontal separator, a drip, a 2 inch sales meter with a .75 inch orifice and a chart recorder all within an earthen dike. The 210bbl tank was disconnected from the dump line. The well was equipped with 8.625 inch casing; 4.5 inch casing, 1.5 inch tubing and a plunger lift system with all valves in the closed position. I took readings with ODNR Honeywell PhD6 meter revealing no H2S at the time of inspection. I placed call to Enervest requesting an update on well and Tim McConaha stated that he would see how soon Knox Energy could plug the well.
0459918942 2015-05-19 Plug / Plug Back No Violations On the day of my inspection following up on NOV- 687342636, I witnessed Berry Well Service retrieve their fish with a 2.375 inch overshot and run their tools down to 5537 feet. Appalachian Well Service was on site and ran down inside 1.5 inch tubing with a 1.5 casing cutter, found free point at 5496 and shot tubing off. BWS pulled tubing free and laid down 1 joint and will set bottom plug tomorrow.

0661930099 2013-10-30 Production Wells No Violations On the day of my inspection, I found no change in the status of the well, I placed a phone call to Terry Miller the pumper for Enervest to ask what was taking place with the well, I was told the well is on the plugging list, no permit to plug has been issued at this time, the two hundred ten barrel tank for the well has developed a hole at the bottom, the pumper told me all the fluid has been removed and the production lines have been disconnected, the pumper also stated the tank is scheduled for removal.
0687342636 2013-04-22 Production Wells Violations Noted On the day of my inspection I reviewed the Division of Oil and Gas production records and found that they were not current for the years of (2008,2009,2010,2011,2012). My inspection at the well site reveals 8.625 casing with 4.5 production casing and 1.5 tubing and gate valve standing from the ground, all the valves at the well head are closed and the well head is painted yellow, After reviewing previous inspections done by inspector Mike Kavage I found the well has a hole in the four inch due to H2S gas, my gas monitor did not detect H2S at the time of my inspection. The requirements to meet the Division of Oil and Gas Standards will be to make the well capable of commercial production within thirty days or plugging is required within ninety days, failure to comply may result in Forfeiture of Bond.

1382209416 2015-05-21 Plug / Plug Back No Violations On the day of my inspection I witnessed Berry Well Service had ripped the 4.5 inch casing at 2910 feet and began pulling casing upon completion they will run 1.5 inch tubing back in the well bore and complete plugging of well tomorrow. All production equipment has been removed.

1431956551 2015-05-18 Plug / Plug Back No Violations On the day of my inspection following up on NOV- 687342636, I witnessed that Berry Well Service had got their tools down to 5517 feet inside the 1.5 inch tubing. BWS speared the 1.5 inch tubing and began working the tubing and pulled 25000#. BWS continued working the tubing up and down and the spear parted at 36000# dropping 2 feet inside the 4.5 inch casing. BWS will get an overshot and retrieve the spear, run their tools back in the tubing and shoot the tubing off and commence with plugging of well.
1511694323 2015-05-20 Plug / Plug Back No Violations On the day of my inspection accompanied by ODNR Inspector Austin Gutridge, I witnessed Berry Well Service and Cemenco Service Inc. was on site. Cemenco began pumping on tubing and it pressured up to 750psi. Berry Well Service pulled 2 joints of 1.5 inch tubing and Cemenco was able to clear the tubing and pumped a 438 foot 40 sack Class A bottom hole cement plug from 5405 feet to 4967 feet. BWS pulled 45 joints of 1.5 inch tubing and Appalachian Well Service perforated the Big Lime with 10 Shots from 4001 feet to 3991 feet. Cemenco pumped a 200 foot 60 sack Class A Big Lime cement plug from 4000 feet to 3800 feet. BWS will pull remainder of the tubing and rip the 4.5 inch casing and remove and run 1.5 inch tubing back in and plug remainder of the well.
1644910346 2015-05-22 Plug / Plug Back No Violations On the day of my inspection I witnessed Berry Well Service had removed 4.5 inch casing and ran 1.5 inch tubing back in the wellbore to 2886 feet and found that the hole had collapsed. BWS pulled 1.5 inch tubing back out, cleared tubing and ran tubing back in the wellbore to 1466 feet. Cemenco was onsite and pumped a 400 foot 114 sack Class A bottom of the surface plug from 1466 feet to 1066 feet. BWS pulled 40 joints of 1.5 inch tubing and Cemenco pumped a 63 sack 200 foot Class A cement Surface plug from 200 feet to surface. BWS pulled remaining 6 joints of 1.5 inch tubing and cement stayed at surface. NOV-687342636 is resolved.
