0609186002 2015-03-12 Production Wells No Violations On the date of inspection I was accompanied by ODNR Inspector Patrick Shreve. I found production equipment to include one 210bbl tank and a horizontal separator within an earthen dike. The well was equipped with a gas plunger lift system, 2.375 inch tubing, 4.5 inch casing, and 8.625 inch casing. Tubing valve is open and production records are up to date.
0632939013 2017-08-22 Final Restoration No Violations One the day of my inspection I found that the wellsite and containment area are now vegetated. Site meets the requirements of ORC 1509.072 (B) for final restoration.
1208737610 2016-11-15 Production Wells No Violations With the conduction of my inspection I discovered the permitted well bore is utilizing a plunger lift system for means of production. The casing program for the well consists of 8.625 inch diameter surface casing, 4.5 inch diameter production casing and 1.5 inch diameter production tubing. The plunger is set up to be operated by a motor valve or with manual valves. Upon my arrival at the well head an odor of H2S could be smelled at the well bore. After inspecting the well head I discovered severe corrosion on the valves that controlled the well and also severe corrosion on the production tubing and production casing above their well heads. I then gathered gas readings with my Industrial Scientific Tango TX1 hydrogen sulfide meter and received H2S readings at 8 ppm around the well head. Production equipment for the well consists of a 210 barrel production tank and a horizontal separator. The production equipment was placed in an earthen dike that measured approximately 40*40 feet with an average height of 2 foot around the perimeter of the dike. Also placed in the earthen dike was the production equipment for permitted well number 34-067-2-0618 (Sayre #2) which consisted of a 140 barrel production tank and a horizontal separator. Over the duration of my inspection of the well head and the production equipment I witnessed a domestic hook up was present coming from the production casing valve. The valves for the domestic hook up were placed in the closed position. Proper identification was placed on the production tank and at the well head. RBDMS indicates is making commercial volumes of production. I will perform status checks to make sure the well is producing.
