1332606018 2010-10-20 Production Wells Violations Noted Received complaint via email from Mike McCormac (Central Office). The complaint was forwarded to Mike McCormac from Asst. Director Rich Milleson on behalf of the landowner (Ms. Dauh). The complainant is concerned that the well is idle and hasnt been in production for several years. My inspection revealed that there has been no recent activity at the tank battery. The weeds have grown up around the oil storage tank and there is no chart on the gas meter. I went to the Dauh residence and met with the Dauhs and explained the procedures that the Division of Mineral Resources Management follows for dealing with idle wells. I then went to the wellhead and conducted an inspection of the wellsite and lease road. There are several areas of erosion and sedimentation on the sloped portions of the lease road ( I took photographs). Inspection of the well head revealed that the valves are rusted and closed. I took several photos of the well head to show the lack of activity at the well head. I checked RBDMS and it indicates that there has been no production reported since 2006. The well is idle and the operator needs to either plug or produce the well in accordance with Ch.1509 ORC. The operator also needs to address the issues of the erosion and sedimentation on the lease road by repairing and stabilizing the eroded areas. The operator needs to conduct the remedial actions by December 6th, 2010. NOTE: While I was at the well site, I could smell what appears to be a slight odor of H2S gas. I didnt have my gas detector with me to confirm if it was H2S gas at this time. The next time I conduct an inspection on this site , I will have a gas detector with me to confirm if the odor is H2S gas or not.
1365702147 2013-04-11 Final Restoration No Violations I am checking this well today for Chief Simmers. My inspection found that this well has been plugged and the production equipment removed. The well site and tank area has been graded and seeded and good vegetation established. I met with the landowner and he advised me that he was satified with the final restoration.
