0350740173 2012-08-20 Drill / Deepen / Reopen No Violations I was on site to check the status of Nomac Drilling Rig # 311. My inspection revealed that Grant Smith Trucking is still moving the rig onto the site. My inspection also revealed that two cranes are being used to put the derrick onto the substructure. They advised me that they are hoping to get the derrick in the air today and spud the hole sometime before tomorrow morning. They are anticipating circulation problems while drilling below the 30 conductor casing, so they are going to have a 1000 barrels of polymer drilling mud and a 1000 barrels of water on site before drilling below the 30 conductor. My inspection revealed no violations at this time.
0501777280 2013-10-11 Preliminary Restoration No Violations I was on location to pass preliminary restoration for this well. The frac has been completed and the drilling equipnment has been removed from the location. At this time preliminary restoration can be passed.
0540917936 2012-07-28 Drill / Deepen / Reopen No Violations I was on site to witness the cementing of the conductor casing. My inspection revealed that Precision Rathole has drilled a 32 hole to a depth of 45 and ran 30 conductor casing to a depth of 45. O-TEX cemented the casing with 100 sacks. NOTE: This site is located in a reclaimed surface mine. Cement never circulated to surface; it stayed approximately 25 from surface. I advised Precision Rathole that they needed to get a ready-mix truck to top off the annular space between the 32 hole and the 30 casing. I advised them that they needed to complete the remedial cement job either tomorrow morning (Sunday) or Monday morning if they couldnt get a ready-mix truck on Sunday morning.

1071102543 2016-02-25 Production Wells No Violations My inspection today found the access road gate locked and the access road in good repair. The pad is fenced and all man doors locked. I found the #3H well has been drilled but not in production. No annular pressure gauge was found on the wellhead tree. Concrete barriers were in place around the wellhead. Well identification number was not found on the well but was on signage at entrance to access road. No production equipment nor tanks have been placed on the pad.
1280739123 2013-07-23 Production Wells No Violations I was on location to do a status check of this well. The well is shut in and currently has no production equipment on location. The well shows no annular pressures of any kind at this time. A new waste water drainage system has been installed on location and the erosion controls have been built up as well. There are no violations at this time.
1351713200 2012-10-31 Production Wells No Violations My inspection found that the lease road was well maintained with a locked gate. The well pad was fenced and contained one well with an assembled christmas tree wellhead onsite. The well contained a large amount of rainwater but it was contained on the pad and there was no water leaving the pad. A Whipstock crew was onsite pumping the rainwater off the pad over the barrier wall at a slow rate to reduce erosion. The well was surrounded by steel fencing and cement barriers. There are no violations at this time
1359652128 2013-01-31 Production Wells No Violations I was on location to inspect the Zimnox pad that was recently fracked the past week. The wellhead is currently surrounded by a cage and has concrete barriers around it. The location has a very large amount of water on it due to the rain and snow melt, a crew is currently on site pumping water off location. Some plastic containment was left on location and the strong winds have blow some against the fence in the corner. I have contacted Chesapeake to let them know that the fence is about to give way and to have someone pick up the containment so that it does not blow off location.
1362433839 2013-03-04 Production Wells No Violations I was on this location to check the status of the cleanup of the plastic containment that was blowing into fences and had the possibility of leaving the location. Currently there is a crew on site collectiong all left over containment. The blow containment has been removed and fixed. Onsite is also 3 frac tanks and a sand seperator for the processing of flowback from the hydrallic fracturing of this well. There are no violations at this time.
1363629649 2013-03-18 Completion Testing No Violations I was informed that flairing on this location was occuring and would take 2 weeks to complete following the hydrallic fraturing of the well. The job and flairing has just been completed and the majority of the equipment removed from the site. Still on site is a Baker frac tank and the Weatherford flowback trailor. A new christmas tree wellhead has been assembled and the well is surrounded by steel fence and cement barriers. There are no violations at this time.

1480512450 2016-11-30 Production Wells No Violations On the date of my inspection I was unable to gain access to the well location. The access road entrance road entrance is blocked. The landowner has parked a truck directly in front of the access gate. I spoke with the landowner about moving the truck and she indicated they could move it later in the day as the only key was with her husband who was at work.

The pad is surrounded by an earthen dike with healthy vegetation and a fence with a locked gate. The pad has 1 unidentified well surrounded by concrete barriers. There is no production equipment on the pad. The wellhead is equipped with a christmas tree valve. A pressure gauge at the top of the valve assembly shows approximately 275 pounds per square inch. This well is listed as a stratigraphic well in RBDMS.
1498655289 2017-06-28 Production Wells No Violations On the date of my inspection I was unable to access the well pad to perform my inspection. The landowner has a truck parked on the lease road and there is no way around the vehicle. She indicated that her husband has the keys with him at work. She also indicated that Chesapeake has sent them a letter of release. I left a message with Chesapeake inquiring their intentions for this permit number which is currently in its second year of inactive status.

The pad is surrounded by an earthen dike with healthy vegetation and a fence with a locked gate. There are small trees growing on the pad near the wellhead that are approximately 8 to 10 feet tall. The pad has 1 unidentified well surrounded by concrete barriers. There is no production equipment on the pad. The wellhead is equipped with a christmas tree valve with chains looped through the valve handles. A pressure gauge at the top of the valve assembly shows approximately 3000 pounds per square inch. This well is listed as being in its second year of inactive status. The owner has recently switched from Chesapeake to Ascent Recources. I called and left a message with Adam Pence with Ascent Resources regarding identification tags and their intentions for this well since it has been idle since 2013.
