0549495582 2014-01-20 Plug / Plug Back No Violations Information for this report was provided by the Enervest Operating Contractor and Formation Cementing Cement Ticket. I was not present during this activity. Berry Well Services used the 1-1/2 inch tubing to place the first plug from 2881 to 2481-feet. They removed the tubing to allow Appalachian Well Services to rig up their perforator to shoot the 4-1/2 at 1820 so they could establish circulation to the surface and ultimately remove the 4-1/2 inch tubing using it to plug the remaining intervals on the way out of the hole. They could not establish circulation and could not pull the casing free at that point after pulling up to 30,000 lbs. Returned the 1-1/2 inch tubing to 1820-feet perfed interval and cemented from that point to 1670-feet. They pulled the tubing out of the hole and Appalachian Well Services shot the casing at 615-feet above the first telegraphed point, 647-feet, determined earlier. Berry was able to pull the 4-1/2 inch casing apart and then set cement from that point through the remaining 6-1/2 inch original bore hole into and 100-feet above the bottom of the 8-5/8-inch surface casing. This provided the 100-feet in/ 100-feet out plug for the bottom of the surface casing. Berry finished the cement job by placing 150-feet plug at the top of the surface casing.

1109669553 2014-01-16 Plug / Plug Back No Violations Appalachian Well Service arrived around 0800 hours. They rigged up their CCL-VDL tool, went to bottom, and began logging from 2910-feet to 500-feet below ground surface. Results of their run confirmed the total depth sounded yesterday. Apparently cement bond was good until from 2910-feet to 1900-feet. Cement started to feather out to 1850-feet, then apparently no cement coverage around the 4-1/2-inch casing. Consulted with Dan Goins about the situation. He suggested that they cement from the bottom and place a 500-feet plug. Then try to shoot the casing at 1850-feet and try to establish circulation to surface. If it is established then theoretically try to pull 4-1/2 inch casing free. Once free, use casing to circulate cement up the backside of the 4-1/2 inch casing to cover the Berea at 1730 to 100-feet above the top of the Berea at 1620-feet. Then complete the 100 in/ 100 out and surface plug using the 4-1/2 inch casing to deliver the cement mix. If there is no circulation to surface then shoot the 4-1/2 inch casing at the Berea and spot cement over the perfs. The rip the 4-1/2 incasing at 647, spot cement over top of ripped 4-1/2 inch casing, do the 100 in / 100 out and then the surface plug from 100-feet below ground surface to surface. Berry could not work Friday. They plan to start plugging next Monday. They also decided to move enough 1-1/2 inch tubing to cover the depth from 1850 to surface just in case they could not establish circulation to surface after shooting at that depth. The tubing was staged at Greasy Ridge. Berry ordered Appalachian Well Service to return on Monday with enough material to perform several shots in the casing. Also ordered Formation to come with enough cement to cover all possibilities.

Well is equipped with 4 1/2 by 2 swedge w/ 2 valve (valve is closed) valve has a 2 plug. Well annulus is below grade and not verifiable if connected to line. 30 feet from well is a drip connected to a 2 line and a meter with a chart. Chart is dated 3-1-10. Chart reads 0 production. 2 line is connected to other wells on the line. House gas is being provided through the 2 line not the well. House line is equipped with a meter, pressure regulator. House line is a 1 1/4 plastic line. Well site is wet, Landowner requests that company wait to plug well until dry. Mr. Humphrey (landowner) confirmed that they are receiving house gas from another well on farm through the 2 line.

1392144240 2014-01-15 Plug / Plug Back No Violations Berry Well Service removed wellhead and sounded the bottom of the well with a bailer on end of wireline. There was no production tubing in the well. Total depth was measured at 2954-feet before they encountered refusal. They tried several times to get passed this point with no luck. Pulled bailer out of the hole. Flapper valve had what appeared to be cement. Berry then attempted to pull 4-1/4 inch casing from hole. Enervest contractor informed us that there was only a very short section of pipe in the well. After Berry applied over 30,000 lbs pull on the casing and it did not move something was definitely wrong. Lowered a ripping tool down hole to determine where the first free point in the 4-1/2 inch casing would be as they brought the device to the surface. The first telegraph point was at 647-feet below ground surface. Contacted Dan Goins about the situation and he suggested that Enervest order a cement bond log to determine extent of cement behind the 4-1/2 casing. Berry order Appalachian Well Service to come to the site early tomorrow to run the log. They also put Formation Cementing on standby for tomorrow also just in case we had enough time. Shut down for the night.

Enervest Contractor will return in the spring and final dress out the surface, remove some gas-metering equipment, and seed the bare surface. FR approval is pending until the above is finished.
1613984286 2010-12-30 Administrative Inspection No Violations Received a call from Jeff Humphrey Re: CN issued to Enervest for idle well. Jeff is interested in owning the well. I informed him that the well has been plugged back to the shale and records do not indicate that the well has been stimulated for production. Casing records are not available to determine how much 4.5 casing is in the well. I informed Jeff of the new DM well ownership requirements. I advised Jeff to think about it and then to contact Enervest regarding purchasing the well.
1756726079 2013-11-05 Production Wells Violations Noted This well is scheduled to be plugged by Enervest soon. They applied for a permit to plug this well. Their plan is to move a service rig to perform some work over on an injection well at Greasy Ridge ERP, and then use this rig to plug two of their wells, this being one of them. At this moment they are still not producing this well and there is still no identification sign at the wellhead. According to 1501:NONE_ /1509:.062A if there is no production, the well shall be plugged. And according to 1501:9-9-05(A)10/1509:23 the wellhead should have attached a sign with at least the following information: current well owner, lease name, well number, permit number, county, and an emergency telephone number. The owner shall install a sign at the wellhead with at least the information listed above.
