0293411690 2014-01-15 Production Wells No Violations I performed a follow up inspection on the above location. There is no oil in the contained area west of the tanks. They will start reclaiming the area on 1/17/2014 if the weather conditions permit.
0336262904 2013-12-17 Production Wells No Violations Sutton Pump was on location and steam cleaning and squeezing the area into a pit. Water trucks were also on location. The tanks have been removed and the contaminated stone and soil has been removed and properly disposed of. The lower location has been mopped up with absorbent pads. Due to the upcoming weather conditions 3-4 of rain fall there will be no dirt excavation. There will be booms installed along the sides of the effective area and the siphon dam will stay in place. EnerVest personal will inspect at various times.
0740378131 2009-05-28 Production Wells No Violations I GPSd both the wellhead and the tank battery for the first time. I continued to walk around the tank battery and the wellhead and found no violations at this time.
1032308175 2012-02-09 Production Wells No Violations On 02/06/2012, DNR Inspector Richard Skidmore completed a production well inspection status check for DNR Inspector Chris Grimm. The lease road, tank battery, and well head locations are in compliance with 1501:9-9-05. Inspection passed.
1268782529 2013-12-20 Production Wells No Violations I performed a status check on the above location. The location looks to be free of oil around the tank battery. I talked to Joe Teppart from Enevest about somebody checking this location during the weekend due to the amount of rain in the forecast and also keep booms and absorbent pads at various locations to catch any oil that might come to surface. And watch the siphoning dam and make adjustments if needed.
1291088150 2013-12-30 Production Wells No Violations I performed a status check on the above location. Booms are still in place siphoning dam is operating. No oil was seen in the diked area.
1388068410 2013-12-16 Production Wells No Violations Joe Teppart from EnerVest called me at 12:15pm and stated that the 100 barrel production tank at the above location lost approximatley 4 ( 1600 gallons ) of oil. When I arrived on location Joe was there he had water trucks called out and a oil truck called to unload the 4 4 of oil remaining in the 100 barrel tank. Barnett Excavation was on location and dug a pit behind the 100 barrel tank to collect what oil that was in the dike. The tank battery is setting on a high elevation and the oil ran west down over the slope to a pasture field. The size of the contaminated area was about 18 yards wide and 60 yard long. We walked the location. Due to the low tempatures the oil was very thick. They dug and lined the two pits with liners and squeeged the oil into the pits. At the bottom of the slope in the pasture field they dug a trench around the continamented area and built a 3 high dike for siphoning a dam with a 8 X 6 PVC pipe for the water to flow through. On 12/17/2013, Sutton Pump will be on location to steam clean the affective area and clean out the tank for removal.
1395670857 2014-03-07 Production Wells No Violations No remediation has been started at this time. I spoke with Joe Teppart from Enervest they are waiting for more warmer weather conditions. They will remove some top soil and repair the disturbed lawn. This well is equipped with 1.5 tubing. There is one 100 barrel tank one horizontal separator. ID signs are posted. No noted violations at this time.
1870309234 2014-01-21 Production Wells No Violations Dike area and siphoning dam has been removed. The area will need to be reclaimed and seeded once the weather conditions change. No violations noted at this time.
1983053361 2015-03-17 Production Wells No Violations I performed a status check on the above location. The tank battery is equipped with one (1) 100 barrel tank and one (1) vertical separator. The well is equipped with 1.5 plunger lift. ID signs are posted. No noted violations at this time.
