0260497729 2007-12-17 Production Wells No Violations I performed a random field inspection. GPSd the wellhead and the tank battery for the first time. The ID signs were posted both at the wellhead and the tank battery. The tank battery was in good shape. There were no violations at this time.
0452317338 2018-04-02 Production Wells No Violations The purpose of my inspection was to conduct a status check on well #2-1606. RBDMS indicated reported production was current. Access to the well was stable. No defined road to the well existed. Correct identification was present at the well head. The 8.625 well head was below grade. The 4.5 production casing was equipped with a plunger lift assembly. Production valves were in the open position. I observed gas leaking from the ground above the well head. The tank battery was shared with 1607, 1605, and 1608. A stable earthen dike that was free of fluid was present around the equipment. Equipment within the dike consisted of two (2) horizontal separators, two (2) 100 bbl production tanks and one (1) 50 bbl brine tank. Wells 1605, 1606 and 1607 produced into one separator. The separator remained in good repair and contained a current 16 day production chart. The production tanks remained in good repair. Correct identification was present. Valves were closed and plugged. Hatch lids were closed. The brine tank remained in good repair. Valves were closed and plugged. Following the inspection I spoke with Enervest Superintendent Larry Lowers. Mr. Lowers reported that he was aware of the leak and was working around weather and ground conditions to make repairs. He continued that he had a crew scheduled to investigate the leak and make repairs by 4/11/2018. I will schedule a follow-up inspection to document the status of repairs.
1092416095 2018-05-22 Production Wells No Violations The purpose of my inspection was to witness casing repair activities on well #2-1606. I met Enervest field superintendent Larry Lower on location. At the time of my inspection the 8.625 had been day lighted and a small crack above the 8.625 head was visible. Sutton Supply was on location to weld patches over the leaks. While welding the second patch, the casing burned through. The thin condition of the casing prevented the second patch from being completed. Mr. Lower communicated with management and reported that the well bore would be overbalanced with production fluid to prevent the escape of gas until a well service company could arrive and repair or replace the top joint of production casing. Mr. Lower stated he would notify DOGRM staff when activities resumed.
1527600946 2018-05-23 Production Wells No Violations The purpose of my inspection was to document the status of well #2-1606. ODNR-DOGRM Northeast District Supervisor Stephen Ochs was present during the inspection. At the time of inspection, the well had been killed with production fluid and remained static. I did not detect any evidence of flow. The owner was waiting on securing a service company to replace the top joint of production casing due to thinning that prevented welding on a casing patch. I will schedule to be present during well service operations.
