0427835703 2006-08-17 Production Wells Violations Noted I received a call from Michelle (EnerVest) regarding a failed stock tank and a 45 bbl crude oil release that was contained in the tank battery dike. I inspected the site. I met with Bart Ray (OEPA) and with Terry Dennis (B&B pumper, 330-802-5945). On my arrival, the oil in the tank battery dike was being sucked up by Arvillas pump truck driver, Ed. I talked on the phone with B&B production supervisor, Mitch, about the clean-up. I requested that all contaminated soil be removed and taken to a landfill or to a treatment facility. The leaking 210 bbl stock tank will be removed.

1007651586 2012-02-17 Production Wells Violations Noted On 2/17/2012, DNR Inspector Richard Skidmore completed a production well inspection status check for DNR Inspector Chris Grimm. My inspection of the tank battery revealed a 20 long, 2 diameter line extending south from the oil/brine storage tank to the dike wall. The end of this 2 line was equipped with an upward facing elbow and short open nipple. There was a two (2) inch ball valve, in the closed position, on the 2 line at the oil/brine storage tank. This valve was not locked. I called Larry Lower of Enervest Operating (216-570-8708) about the issue and asked him to have a valve and/or cap put on the open nipple and a lock put on the valve to prevent any accidental spills. I will do a followup inspection by 02/24/2012.
