0089560509 2017-07-11 Final Restoration No Violations This well was plugged and abandoned on 9/27/2016 and Final Restoration was due by January 2017. My inspection today found that the dike and fencing are still present around the former tank battery. There is a 2 flow/sales line riser on the north side of the battery that needs cut off and capped. The location of the former wellhead is graded and the lease road remains and is part of a sand and gravel operation. I spoke with Tim McConaha (Enervest) and he told me he would get somebody to take care of the tank battery in the next couple of weeks and that he will get the land department to execute the waiver (Form 5) for the flow/sales lines being left buried. I will check back later on progress.
0117917038 2016-09-22 Plug / Plug Back No Violations I was on location to witness plugging operations. Chipco, LLC was the plugging and cement contractor and J Hawk was hauling water. Chipco spotted 2 outside diameter (OD) tubing at a depth of 4133 and cleared the tubing. They then mixed and pumped 6.3 barrels (30 sacks) of cement slurry and displaced the slurry to 3733 with 9.3 barrels of water. The tubing was pulled and the pipe trailer was spotted. Chipco ran in the 4.5 casing with the casing knife and jars and ripped the casing at a depth of 2545. The top of the lubricator was removed and the relief valve on the base was disconnected in order to get the travelling block on the top collar of the pup joint. Chipco pulled up on the casing and it was free. The fluid weight on the backside u-tubed into the 4.5 casing and forced a column of gas and oil to blowout the relief valve port. The lubricator was reassembled and the valve was connected to the flow line to the steel pit. Less than one (1) barrel of oil was released and blew against the side of the cement bulk truck keeping the release on the immediate site. A berm was immediately constructed on the north side of location to prevent migration of the release. I had Enervest personnel bring Peat-sorb and pads to location to capture the oil. The ripping tool was pulled from the hole and a 30 joint of casing was pulled to 2515 and Chipco rigged up to set the Big Lime plug. They established circulation and then mixed and pumped 19 barrels of cement slurry (90 sacks). The slurry was displaced to 2215 with 35.5 barrels of water. During displacement, old drilling mud circulated up the annulus and was the primary cause of the blowout. Chipco pulled 25 joints of 4.5 casing to get above the cement and the well was packed off and shut-in until the following morning. The cementing equipment was wiped down and moved and the free product was locked up with the Peat-sorb. The impacted soil and peat were bladed off and stockpiled for removal and proper disposal. No violations.
0277306303 2016-09-21 Plug / Plug Back No Violations I was on location to check the status of Chipco, LLC moving in to plug and abandon this well. Chipco built location and got the rig spotted over the hole. They ran in the hole with the ripping tool to verify that the 4.5 production casing was unobstructed prior to running tubing. They landed the tool in fill-up at a depth of 4125. There is 4227 of casing in the hole and the perforations are from 4112-4157. Chipco will spot tubing as deep as possible and start plugging the well tomorrow. No violations.

1422634793 2015-01-14 UrbanProduction Wells Violations Noted I received an e-mail from Inspector Paul Greco concerning a well located at IM-1017 (Rainbow Sand & Gravel) on 1/7/15. I contacted the site Forman, Dale Gotshell, and I met him to inspect the site today. According to Dale, the pumping unit, rods, tubing, and surface equipment at the tank battery were removed several years ago and the well has been idle since then. Aerial photos show that the equipment was removed between November of 2011 and April of 2012. I inspected the wellhead and found the 4.5 casing just above ground level and equipped with a casing valve that was closed. The fencing, a belt guard, and other miscellaneous parts were shoved into a pile to the west of the wellhead and are being overgrown by weeds. An inspection of the tank battery found that the tanks and production equipment have been removed. Fencing still surrounds the former tank battery and risers for the flow and sales lines are still present. A very faint smell of gas was noticed near the riser pipes and I could not determine if the gas was emanating from the flow line or back-feeding from the sales line to the west. The last commercial production reported for this well was 151MCF in 2007 by Range Resources and the well has not produced commercial quantities of oil or natural gas since that time. The required identification is not posted at the wellhead or at the former tank battery. Based on the lack of commercial production reported since 2007 and the removal of all the production equipment from the wellhead and tank battery 2-3 years ago, Enervest Operating will, within 180 days, obtain a plugging permit as required by section 1509:13 of the Ohio Revised Code and properly plug and abandon the well in accordance with section 1501:9-11 of the Ohio Administrative Code. Failure to comply with this notice may result in further administrative action.
1465943412 2016-09-27 Plug / Plug Back No Violations I was on location to witness the pumping of the top plug on this well. The cement plug was tagged at 298. Tubing was ran in and Chip Co pumped 90 sacks of class A cement 14.8 ppg to surface there was good cement returns. Tubing was tripped out and well was topped off. The cement stayed at the surface. The top cap with the ID will be welded on at the surface because this is in a sand and gravel pit.
1499782700 2017-07-11 UrbanProduction Wells No Violations Follow-up to Compliance Notice #1422634793 issued on 1/14/2015 for ID violations and that the well should be plugged and abandoned. The well was plugged and abandoned on 9/27/2016 and satisfied the outstanding violations. This Compliance Notice can be closed.
