0219038552 2011-09-07 Production Wells No Violations The clean up continues. The beaver dam area is 95% cleaned up complete. The syphon dam is still installed. The area inside the dike has started to be cleaned up. This NOV continues.
0573272267 2011-08-23 Production Wells No Violations On site to assist inspector Carl Roberts in assessing and remediating oil spill in wetland area. Operator had a flow line break under an area flooded by a beaver dam. Water depth 1 to 4 feet. I had advised Carl over the phone to have company start with booming off affected area to contain spill, have company obtain oil skimmers, additional booms and leaf blowers to contain and recover oil. Carl and I discussed the feasibility of a siphon dam. Oil will need to be recovered manually. When I arrived on site, EPA had also arrived. EPA advised to spray peat sorb on oil floating on the water to lock it up and break it down. No recovery was suggested at this time by the EPA. Affected area by the free crude oil was 100 yards by 150 yards. initial estimated amount of spill 2Bbls.
0796725001 2011-08-25 Production Wells No Violations I performed a NOV follow-up. Kurt Kollar from the Ohio EPA is on the location and is in charge of the clean-up in and around the beaver dam area. Enervest has a 50 barrel plastic tank, hoses and water pumps. The crew of 8 Enervest people are using the hoses to clean the oil from around the brush, weeds in the beaver dam area and pushing the oil to a central area for getting vaccumed up. Then the fluid is pumped into the plastic tank. The clean-up team has recovered approximately 1/2 barrel of oil. This NOV continues.
1314268960 2011-08-23 Production Wells Violations Noted I received a call from Joe Tippirt(330-323-9383) Enervest about a sales line spill into a beaver creek/dam area. Joe had also called the Ohio EPA. The sales line feeds 5 wells. I arrived on location 30 minutes later. Joe and I checked the damage. There was estimate of a 3 barrel oil spill. The beaver dam had cause the water in the creek area to back up 150 yards by 100 yards. But the spill was concentrated in a 40 feet by 100 feet area due to all the brush in the area. The clean up started. Ohio EPA arrived, Matt Balser(216-789-9281). It was decided to blow peat into the brush and grasses. The job would start tomorrow. Peat was laid down around the contaminated area. This NOV continues.
1328815530 2018-12-20 Production Wells No Violations Status check inspection of this Clinton Sandstone well. My inspection found the wellhead equipped with 2.375 rods and tubing and an electric powered pumping unit. The wellhead and unit are diked. This well produces to a diked tank battery on location and is shared with the Brosius #2 (PN# 21057). The battery contains shared equipment consisting of two (2) 100 barrel tanks, a 35 barrel poly tank, and a horizontal separator. The required ID is posted at the tank battery and the wellhead is properly identified. No violations.
1344880117 2012-08-03 Production Wells No Violations FOLLOW UP TO NOV #1369313134. On location to check the status of an oil spill cleanup from 8/23/11. The impacted area has been cleaned up and remediated. Well is equipped with a pumping unit. There is a small gas leak in the slips between the tubing and 4.5 casing and I called Kyle Ahbel (Enervest) and left him a message about the gas leak. The tank battery has sufficient containment and contains a separator, a 50 barrel poly tank, and two 100-barrel stock tanks. A siphon dam is located outside the western side of the tank battery. The lease road is in good condition. No violations.

2068236880 2011-08-24 Production Wells No Violations EPA and company on site at the time of inspection. EPA changed method of clean up. Operator was using leaf blowers to move oil into a recovery zone. Siphon dam at been constructed north east corner of the beaver dam. Operator had booms in place before the siphon dam. EPA and Operator had constructed a decanter. Operator was using a skimmer to draw oil into decanter and separate the oil and water. Clean up efforts to continue. Operator will have personnel on location during the night.
