0000000538 | 2002-10-29 | Production Wells | Violations Noted | See Case # 02-066. Received an oil well odor complaint from Vicky Dunford (330-677-9500), 3767 Ranfield Rd., Kent, OH 44240. 10-29-02 Inspected the site. No odor was present. Issued N.O.V. for oily rags and stained soil around the wellhead and for detached belt guard on the pumping unit. Contacted John Fleming (Great Lakes) and informed him of the N.O.V. He told me that Great Lakes had repaired a sales line leak recently. This probably was the source of the odor complaint. Called Vicky Dunford and informed her of the findings. |
0071615724 | 2015-08-21 | Plug / Plug Back | No Violations | I was on location to check the site following plugging operations. Bishop Well Services has rigged down, the pit has been closed, and the surface casing has been cut off and buried. They are currently solidifying the circulated mud and fluids in the half-round tank with sawdust. Sutton Pump and Supply is lining the back of a dump truck to haul the solidified material to a landfill for disposal. No violations. |
0107694925 | 2011-08-10 | Production Wells | No Violations | I received a call from Mr. Chester Rahods of Ranfield road again about the fence around the wellhead. I arrived on the scene 40 minutes later. I called Tom Basick(330-575-9248) about installed a guard around the belts on the pumping unit and getting the fence either fixed and locked or installing a new fence with a lock. This area has kids playing and needs to have a good fence around the wellhead. The well was shut in within 2 hours. He stated that he will get started on the fence and guard. This NOV continues. |
0129600404 | 2013-08-12 | UrbanProduction Wells | No Violations | I inspected this well in regards to Complaint #4387 concerning the well not producing and the overgrown surface facilities. I called the landowner and left a message that would be out to inspect the well. My inspection found that the tank battery was fenced and diked and contained a 210 barrel tank and a 50 barrel poly tank. I was unable to access the battery due to the excessive overgrowth of weeds and brush and could not determine the nature of the production unit. I inspected the pumping unit and found that the unit was diked and fenced. There was an ID sign posted but the information was worn away. I could not enter the fenced area due to excessive overgrowth. The polished rod appeared heavily rusted which indicates that the well is not producing. A check of records indicates that inspector Carl Roberts had Enervest fix the pumping unit and fence in July-August, 2011 as well as produce the well. Production records show 39 barrels of oil produced in 2011 and no gas. I contacted Enervest and asked them to take care of the weeds at this location as soon as possible and get the well placed back in production. I will check back later to see if the weeds have been taken care of, or I will issue a compliance notice. |
0226133291 | 2015-08-13 | Plug / Plug Back | No Violations | I was on location to check the status of plugging operations of this Clinton Sandstone well (Complaint #4387). Bishop Well Services has unseated and moved the pumping unit off of the wellhead. The service rig is spotted on the west side of the wellhead and they will begin pulling rods and tubing tomorrow morning. No violations. |
0671262077 | 2015-08-31 | Plug / Plug Back | No Violations | I was on site to inspect the location following plugging operations. All plugging equipment has been removed and the area around the wellhead and access road have been graded. The surface equipment in the tank battery is still in place and needs to be removed and the area reclaimed. Final restoration is due by the third week of November. No violations. |
0795094410 | 2015-08-18 | Plug / Plug Back | No Violations | I was on location with Inspector Shannon Mullins, Bishop Well Services, J-Hawk Water, and Petroset Cementing to witness plugging operations. Petroset rigged up to the 2.735 tubing and pumped 110 barrels of water in an attempt to establish circulation. It was decided to pump the bottom plug and Petroset mixed and pumped 7.3 barrels of Class A cement. The cement was displaced to 3963 with 15 barrels of water and the hole circulated during displacement. Bishop pulled the tubing from the hole, ran in with the knife tool, and ripped the 4.5 production casing at a depth of 2973. They pulled 7 joints to a depth of 2750, packed in the wellhead, and shut down for the afternoon. No violations. See plug report for more details on this job. |
0858739305 | 2014-08-17 | Plug / Plug Back | No Violations | I was on location to check the status of plugging operations. Sutton Pump & Supply was on location and steaming out the 2.375 tubing prior to Bishop Well Services running it back into the hole to set the bottom plug. All oil and paraffin was being collected in a contained steel tank at the end of the tubing rack and J-Hawk Water Service hauled the residual liquid for disposal. Bishop tallied the tubing and they will spot the bottom of the pipe at a depth of 4,318 (21 below the bottom perforation). The bottom plug will be set tomorrow morning (8/18/15). No violations. |

1261753059 | 2014-05-28 | UrbanProduction Wells | Violations Noted | I inspected this well to follow up on idle well complaint #4387. I initially inspected this well on 8/12/2013 and contacted Woody McDaniel (Enervest) to have someone take care of the weeds and place the well back in production. My inspection today found that none of the issues have been addressed. The wellhead and tank battery are still badly overgrown and inundated with tall weeds. The required ID is worn off and not legible. Production records indicate that there has been no commercial production from this well for 2 consecutive reporting periods, the last being in 2011, and is the only production of record since 2008. The required ID and general houskeeping (weeds and overgrowth) need to be taken care of immediately. Enervest Operating LLC will, within 180 days, obtain a plugging permit as required by section 1509:13 of the Ohio Revised Code and properly plug and abandon the well in accordance with section 1501:9-11 of the Ohio Administrative Code. Failure to comply with this notice may result in further administrative action including issuance of a Chiefs Order. |
1269864776 | 2011-08-15 | Production Wells | No Violations | The fence around the wellhead has been taken down. They will replaced the old fence yet today. This NOV continues. |
1311271080 | 2011-07-21 | Production Wells | Violations Noted | I received a call from the Uniontown office about a complaint on this well. Complaint number 3246. Mr. Chester Cahods(330-903-3360) of 3753 Ranfield road called about this well being idle. I inspected the well and tank battery and found the well has not been producing. Part of the fence around the wellhead was down. The weeds were 5 feet high. Production reports show no sign of production since 2008. I called Tom Basick(330-575-9248) of Enervest about this well. Tom stated there were production line leaks. He will try and get a workover rig out in the next couple of weeks to get this well back into production and clean up the weeds and perform general house cleaning. This NOV continues. |
1311793405 | 2011-07-27 | Production Wells | No Violations | The wellhead and the tank battery area were cleaned up. The weeds cut and the fence around the wellhead up. The NOV continues. |
1313599389 | 2011-08-16 | Production Wells | No Violations | The fence with barbed wire on top has been completed. There still was gas leaking from a pop off by the wellhead. I called Bob Worstall to see what he thought. This wells pop off was repaired and gas is now going to Mr Chester Rhoads house. The ID sign is posted. The gate is locked at the tank battery. The fence is locked at the wellhead. This NOV is resolved. |
1506802095 | 2011-08-15 | Production Wells | No Violations | I performed a NOV followup on this NOV. The old fence was torn down. Progress is being made. This NOV continues. |
1549089617 | 2015-08-14 | Plug / Plug Back | No Violations | I was on location to check the status of plugging operations. Bishop Well Services was currently pulling the pumping rods from the tubing. I gave them the plugging depths and discussed the procedures with the operator. No violations. |
1584358305 | 2015-09-25 | Final Restoration | No Violations | I was on location to follow-up on idle well Complaint #4387 and Compliance Notice #1261753059. The well was properly plugged and abandoned on 8/20/2015. My inspection today found that the tank battery has been dismantled and all the production equipment has been removed from location. The wellhead area and lease road have been graded and seeded. The tank battery dike has been removed and the area has been graded. Final Restoration is due the third week of November and will be approved when I receive the landowner waiver to leave the flow line buried. This Compliance Notice has been satisfied and the idle well Complaint should be closed. No violations. |
1862026642 | 2011-08-02 | Production Wells | No Violations | The pumping unit is operational. Parts of the fence around the wellhead are down. The gate to the tank battery was open. I called Tom Basick(330-575-9248) about the fence. He would have the fences fixed. This NOV continues. |
2046427839 | 2015-08-20 | Plug / Plug Back | No Violations | I was on location to witness the final plug being set in this well. After setting the Berea Sandstone plug on 8/19/15, the 4.5 production casing became hung in the hole at a depth of 469. I advised Dave Bishop (phone) to allow the cement to set up overnight and we would verify the top of cement today, rip the remaining casing, and top the hole off. Today, the top of the cement was tagged at 236 and the 4.5 casing was ripped at a depth of 167. 2.375 tubing was spotted at 155 Sidley Concrete grouted the top of the 8.625 surface casing in with 3.5 cubic yards of 9 sack grout. The tubing was pulled and the hole was topped off. The casing will be cut off and the ID plate will be attached tomorrow. See plug report for more details on this job. No violations. |
