0695642006 2019-02-08 UrbanProduction Wells Violations Noted I was on location today after the operator (Enervest Operating, LLC) reported that the 8-5/8 X 4 1/2 annulus was found to be over-pressurized following an annular pressure request check by Orphan Well Inspector Kevin Perry on 2/7/19. I was on location with Inspector Perry, Mark Crookham (Enervest), and Pyramid Pipeline to gauge and mitigate the pressure at the wellhead and on the annulus. A gauge indicated that the 4-1/2 production casing and the 8-5/8 surface casing and 4 1/2 annulus were equalized at 440psi. Completion records indicate that the surface casing seat is at a depth of 411 measured from ground level (GL). This casing seat is considered over-pressurized when the pressure measured at surface exceeds 124.5psi (.303 x 411) and the pressure found at the seat is currently in violation of 1501:9-1-08(N)(2) of the Ohio Administrative Code. Pyramid killed the pressure on the production casing with salt water and the residual pressure was bled off to the tank battery. The 8-5/8 annular port was plumbed to the flow line and the pressure was slowly bled off to 0psi. The casing port valve was left closed and the annular port valve was left open to keep pressure off of the annulus. The equalization of pressure on the production casing and annulus demonstrates that the 4-1/2 production casing is currently defective. I spoke with Doug Beaghle (Northern Ohio Operations Superintendent) on 2/12/19 about the defective casing issue and informed him that a Compliance Notice would be issued. Enervest Operating, LLC will be afforded 30 days from the discovery date of the defect to assess remedial options and submit a plan to 1) repair the defective casing, 2) plug and abandon the well, or 3) perform another activity approved by the Division. Failure to comply with this notice may result in further enforcement action.
1231594700 2019-02-13 UrbanProduction Wells No Violations Follow-up to Compliance Notice #695642006 concerning this well that was found to be equalized at 440psi on the 4.5 production casing and on the annulus between the 8.625 surface casing and 4.5 casing on 2/8/19. My inspection today found the valve on the annular port open and the gauge that was installed reads zero (0) psi. There is currently no pressure on the annulus. This violation will continue until the casing is repaired or until the well is properly plugged and abandoned.

My inspection today found the valve on the annular port is open and the gauge that was installed reads zero (0) psi. There is currently no pressure on the annulus. I inspected the tank battery for this well and found it fenced and diked. The battery contains one (1) 100 barrel tank, a 35 barrel poly tank, and a horizontal separator. The interior of the battery is becoming overgrown with small trees and identification is posted on the tank. Production records show that the well has not produced since 2010.
