0219126367 2002-12-12 Production Wells No Violations Follow up inspection of the ongoing flowline spill remediation being conducted by Wade Pol Contracting (330-325-7177) for Equitable Production (Alan Haggerty 304-269-2061). The leaking flowline has been replaced and the well is back in line. The impacted drain tile has now been completely replaced and backgraded. The impacted soil and vegetation surrounding the pond have been removed and the pond outlet has been redesigned in order to contain any residual oil in the pond. The project is approximately 90% completed. Remaining work involves restoring the drain tile trench and monitoring the pond for any sheen. This work will carry over into the spring.

0946336945 2018-02-07 Production Wells No Violations Status check inspection of this Clinton Sandstone well. My inspection found the wellhead equipped with 2.375 rods and tubing and operating with the aid of an electric powered pumping unit. This well produces to a diked tank battery to the east and is shared with the Graham #3 (PN# 22227). The battery contains two (2) 210 barrel tanks, one (1) shared 35 barrel poly tank, and two (2) horizontal separators with the associated meter runs. Identification is posted at the wellhead and at the tank battery. No violations.
1024800646 2003-03-21 Production Wells Violations Noted As noted in previous inspections, the majority of the remedial action at this spill has occurred. The site will be monitored for any sheen or free oil on the pond. At this time there is a light sheen on approximately 10% of the pond surface. I contacted Wade Pol and suggested that they remove the old booms on the pond, skim the surface with new booms and rake any impacted organic material from the pond edge.

On this date there was a light sheen on the Craig pond and the area where the drain tile was replaced and the vegetation removed from around the pond, still needs reclaimed. Wade Pol is still under contract to remediate the spill for Equitable. The well however, has been sold to Great Lakes Energy.
1446516327 2003-05-28 Production Wells Violations Noted The pond remediation from the oil spill last winter is nearly completed. As soon as the vegetation is better established, the violation will be corrected.
1688012649 2002-12-06 Production Wells No Violations NOV Follow up inspection on spill remediation activities. On this date, employees of Wade Pol Contracting were removing the old clay tile drain tile and any impacted soil and replacing the tile with slotted plastic. The impacted soil was transported to and disposed at the Petrocell facility in Seville, Ohio. The leaking flowline will be replaced and pond remediation will continue.
1950446420 2002-11-27 Production Wells Violations Noted Inspector Worstall received a call from John Fleming (330-296-7611) and Doug Horning (330-418-4877) with Great Lake Energy reporting a oil spill adjacent to Great Lakes Craig #1 well (P-0356) in Atwater Township, Portage County. Great Lakes checked their wells in the area including the Craig #1 and could find no source on their leases. Further investigation by Great Lakes indicated that the oil source appeared to be on the neighboring property to the south. The Graham lease directly south of Craig, has three wells which are owned by Equitable and operated by Wade Pol (330-325-7177). It appears that a flowline leak adjacent to the Graham #2 well (P-2037) entered a drain tile, flowed north and impacted a 1/4 acre, kidney shaped pond on the Craig property. I arrived on site at 14:00 and found a roustabout crew and a vac truck attempting to recover crude oil from the pond surface and from the drain tile. The flowline leak had been repaired and the well was shut in. The field tile had been exposed at three separate locations, all of which contained crude oil. Booms had been placed at the pond inlet and outlet and across the west end of the pond. It did not appear that any significant quanties of oil left the pond and impacted the adjoining marsh. Wade Pol indicated that he intended to continue recovery operations through the holiday weekend. Before leaving the site, I spoke with the impacted proeprty owner (Mr. Craig) and apprised him of the remediation plans. I will return on December 2 to monitor cleanup activity.
