0094795427 2009-05-04 Production Wells No Violations I talked to Mitch Fitch regarding the spill remediation around this wellhead. The contractor is scheduled to arrive tomorrow morning to remove, seed and mulch the impacted area. He is also scheduled to install a chain link fence around the wellhead.
0315127269 2015-11-02 Final Restoration No Violations Follow-up to Compliance Notice #499848845. The well was properly plugged and abandoned on 10/22/15. My inspection today found the location in good order and incorporated into an agricultural field. The wellhead equipment has been removed. The lease road and production equipment remain in use for the production of the H&I #s 1 and 3 (PN#s 22358 & 22360) and a landowner waiver is not required at this time. The Compliance Notice has been satisfied. No violations.
0337604651 2014-10-23 Plug / Plug Back No Violations I was on location to check the status of this well that was plugged yesterday (10/22/14) under the supervision of Inspector John Fleming. Dave Little (KDA Contracting) was excavating soil, that was affected during plugging operations, from around the wellhead and placing it in a half-round tank for disposal. I inspected the 8.625 surface casing and the cement had dropped 10-12 below grade. I asked Dave to top off the hole with bagged concrete mix after the casing is cut off. No violations.
0477386954 2010-09-17 Production Wells No Violations The landowner, Joe Alio, submitted a written complaint to Columbus concerning the condition of this well and tank battery. Some of the issues raised in the complaint were brought to my attention in May 2009 and were corrected within 30 days. The photos included in the recent complaint were not dated but reflect the site conditions at that time. This inspection on September 17, 2010 did not identify any violations or concerns. The fence was in place around the wellhead and the gate was locked. The ground vegetation was established and the trees adjacent to the tank battery displayed no signs of stress. There is no evidence that the brine vault is or has been leaking. The lease road is stoned with a slightly elevated center strip. The road might be difficult for a car but was easily passable for a truck. Refer to the file for photographs taken this date for comparison to the complaint photos.
0499848845 2012-04-25 Production Wells Violations Noted I inspected this well to follow up on idle well complaint #4670. Mr. Aloi states that the well has been idle for 2 years and he would like it off of his property. I met with the landowner on 8/20/2012 pertaining to a flow line leak and he informed me at that time that the rods and tubing had been pulled in the prior 3-4 months which would put the last possible production date in April of 2012. My inspection of the location today found that the well was not in production. The well was equipped with an electric pumping unit that was disconnected from the wellhead. The wellhead consisted of 4.5 casing equipped with a tubing head and a bull plug in the top of the head. The casing and tubing lines were disconnected from the flow line. The tank battery on location processes production from this well and permit #s 22358 and 22360 that are all part of the same 130 acre drilling unit. A check of production records indicates that this well produced in 2013 and is suspect due to the fact that 3 wells produce to the same tank battery through 1 horizontal separator. Information obtained during my inspection on August 20th of 2012 suggests that the 4.5casing is either damaged, parted, or defective. At this time, I was told that the 4.5 casing rotated when attempting to remove the wellhead for servicing and they were trying to determine how to fix the problem. That was nearly 2 years ago and the well status remains the same. Enervest Operating will, within 30 days, 1) demonstrate that the casing is not defective and place the well back in production; 2) determine the nature of the defect, repair the casing, and place the well back in production, or; 3) properly plug and abandon the well in accordance with OAC 1501:9-11 within 180 days of receipt of this notice.
0622752872 2014-07-01 Production Wells Violations Noted I met with Northeast Region Supervisor Tom Hill and landowner Joe Aloi to follow-up on Compliance Notice #499848845 and complaint #4670. We inspected the well and found no hazards or anything leaking. The well is shut-in and idle. It has not produced commercially in over 2 years. This complaint and compliance issue continues.
0908095463 2009-05-07 Production Wells No Violations The quality of the remediation work completed to date is poor. Additional soil needs to be removed around the wellhead and ruts created by the service rig and related equipment need to be filled and graded. If the work is not completed by the next site visit, a NOV will be issued.
0973326153 2013-07-25 Production Wells No Violations I was on location to follow up on a call I received Monday (7/22/13) from Woody McDaniel (Enervest) to report that they had overflowed a tank at this location. He informed me that a timer on the pumping unit malfunctioned and pumped continuously over the previous weekend spilling 0.5 barrels of oil inside the dike. Some of the oil migrated under the dike wall and was cleaned up and hauled away. He told me they were going to rebuild the bad section of the dike and get it cleaned up. My inspection found the site in good condition. There was no oil or staining on the ground inside or outside of the dike from the spill. The area east of the dike had been scraped and a small collection area was built with a siphon dam to catch any residual oil following precipitation events. Booms were placed downslope of the siphon dam as a precaution. I inspected the production equipment and found a 2 flow line running into the separator that appeared to have a small leak below the gravel inside the dike. I notified the pumper and he told me he would be over to fix it. No violations.
1168896012 2014-10-20 Plug / Plug Back No Violations I was on location to witness plugging operations. Petroset Cementing and KDA Contracting were on location to conduct the plugging. Petroset rigged up and attempted to load the hole. Petroset pressured up to 2000psi and could not pump on the well. A salt ring or paraffin build-up likely formed in the casing and would need circulated or drilled out. KDA ran in the whole with the 30 ripping tool and sat down on an obstruction at a depth of 3395 (1055 above where the bottom plug needs to be). They beat on the obstruction without success. I told KDA they would nee to run tubing into the hole with a collar on the bottom that had teeth cut in it to try and drill through the obstruction and circulate on it with freshwater.
1243979494 2009-06-01 Production Wells No Violations The affected area near the wellhead has been remediated, seeded and mulched. The chain link fence has also been placed around the pumping unit.
1346087673 2012-08-20 Production Wells No Violations I received a call from Mr. Tony Aoli about an oil spill/flow line leak at a well on the back of his property. I met with Mr. Aoli on location and inspected the oil and brine leak from the flow line. The impacted soil covered an area of approximately 20 x 6. I called Kyle Ahbel (Enervest) and informed of the leak and he was already aware of it and had shut off the flow line. I told Kyle that the impacted soil would need to be removed, replaced with clean top soil, and be seeded and mulched. The flow line was coming from two wells to the north #s 2358 & 2360 that share a common tank battery with this well. The well at this location was shut in and the rods and tubing had been pulled 3-4 months ago according to the landowner. I asked Kyle about the status of this well and he told me that the rods had frozen up inside the tubing and had to be pulled and cut. He said they tried to unscrew the wellhead and the 4.5 casing turned in the hole. They are currently trying to figure out how to fix this situation.
1347558899 2012-08-23 Production Wells No Violations I arrived on location to follow up on NOV # 1346087673. Enervest was on location to repair the broken flow line, remove and replace the impacted soil, and seed and mulch the new topsoil. The impacted soil was removed to a dump truck and the broken flowline was repaired with a clamp. New topsoil was spread, compacted, seeded, and then mulched with straw. This violation has been resolved and the impacted area has been reclaimed.
1371057738 2013-05-03 Production Wells No Violations I was on location at the request of Woody McDaniel (Enervest) to witness the cleaning out and removal of a brine vault inside the tank battery. According to Woody, the landowner has been blocking access to the lease road for quite some time. Recent rains had caused the diked area inside the tank battery to fill up with water and Enervest got a court order to access the location, remove the water, and work on removing the vault. Scott Miller was on location with J. Hawk water hauling and Sutton Pump. Scott excavated the saturated sludge from the bottom of the vault to haul away and Sutton steamed the inside of the vault. The water was sucked up by J. Hawk for hauling. The vault was sufficiently cleaned and Scott used his excavator to crush the vault walls into the bottom of the hole. Their plans were to fill in the rest of the hole with dirt and topsoil, shrink the tank battery footprint and rebuild the dike. The vault has been properly removed from service. No violations.
1408385963 2014-04-25 Administrative Inspection No Violations Administrative follow-up on Compliance Notice #499848845 and complaint #4670. I have received confirmation from Enervest Operating that the well in question will be properly plugged and abandoned during the fiscal year 2015. The well is shut-in, secured by chain link fencing and the gate is locked. The well is not leaking and is not causing any adverse environmental impact in its current condition. I have extended the required date to plug the well to 07/20/2015.
1418667355 2014-10-21 Plug / Plug Back No Violations I was on location to inspect the plugging of the above well KDA and Petroset Cementing was on location to perform the plugging. KDA ran a4468 of 2.625 upset tubing and tagged the plug at 4197 the top perf is at 4372 there is 175 of cement above the top perf. Ripped 4.5 casing at 3264. Started pulling casing well flowed. Had to shut down because of gas fumes entering the air intakes of the truck motors. When well died down we moved Petroset to a different area due to the flow back tank. See Plugging report for details.
1454344727 2014-10-16 Plug / Plug Back No Violations I was on location on 10/16/14 to inspect rigging up operations. Woody McDaniel (Enervest) and KDA Contracting were on location with the rig spotted over the hole. The 8.625 wellhead was excavated and found 4 below grade. It was determined that the 4.5 casing was not damaged as originally suspected when it rotated during service operations in 2012. A 4.5 riser was unthreading at the casing slips due to a cross-threaded 4.5 wellhead. I approved bullheading the bottom plug and plugging operations will begin Monday morning (10/20/14).

1865587676 2009-05-01 Production Wells No Violations I received a complaint from the landowner, Joe Alio about the condition around the wellhead head and the tank battery. I found there was surface impact of old crude oil and parafin around the wellhead and extending approximately 30 feet southeast of the well. The pumper had placed sorbent pads on the spill some time ago and did nothing to correct the problem. In addition to the oil spill, there were old rods, pallets and operating debris in the tank battery and adjacent to the well. I called and met with Mitch Fitch on location and discussed correcting these violations and the concerns of the landowner. According to Mitch, Enervest did a stripping job last fall. I assume that the surface spills occurred on or before that activity. If the work is not completed in a timely manner, a violation will be issued.
