0572665404 2016-03-08 Production Wells No Violations I was on location this morning to oversee clean-up operations. Enervest personnel and Scott Miller Contracting (SMC) were on location to proceed with remedial operation. I instructed SMC to dig an interceptor trench on the south side of the Kurz #1 tank. A 10 X 2 trench was excavated down to the glacial clay (2 deep). I examined the excavations and the material was found unaffected by the spill. SMC flushed the affected area around and under the tanks with 1,500 gallons of hot water. The released product and water were being vacuumed at the sump under the Kurz #1 tank by an Enervest water truck. Clean water and a small amount of crude oil began seeping into the trench during operations and the trench was functioning properly. Pads will be used to contain and dispose of any residual product collected in the trench. I also asked Enervest to build up the containment dike walls on the east, west, and south sides of the tank battery while they had the mini-excavator on location. They instructed SMC to do so, and they began building up the walls with good clay. The trench will remain open until and monitored until no residual oil is leaching from the affected area and pads will be changed out as necessary. I will continue to monitor the location for any issues.
1066063754 2015-06-26 Production Wells No Violations I conducted a status check inspection of this well. The wellhead is equipped and operating with the aid of an electric pumping unit with 70psi on the tubing and 4.5 casing. The well produces to a diked tank battery, to the south, across Streeter Road and is shared by the Kurz #1 (22430). The battery contains two (2) 210 barrel tanks, one (1) 50 barrel poly tank, two (2) horizontal separators and the associated meter run. The proper ID is posted at the wellhead and tank battery. No violations.

1457454511 2016-03-07 Production Wells No Violations I received a call from Kyle Ahbel (Enervest) at 11:06am to report an oil release at this tank battery. He told me the timer on the pumping unit failed and the well pumped over the weekend and overflowed 1-2 barrels of oil out of the a 210 barrel tank. I notified the Emergency Response Regional Manager, Terry Whitehair, of the reported release. I arrived on location at 12:15 to assess the spill. My assessment found the release is contained within the tank battery and has affected an area measuring 25 X 12 around and below the two (2) 210 barrel tanks located in the battery. The northern tank collects production from this well (Kurz #3) and was over-flowed. The southern tank collects production fluids from the Kurz #1 (PN#22430). The release has flowed from north to south and has collected behind the support beam on the south side of the Kurz #1 tank. Oil is being vacuumed from a sump, behind the beam, by an Enervest water truck. I spoke with Kyle Ahbel (Enervest) and told him that the affected area would need to be steam cleaned, at minimum, and that the tanks may need to be pulled based on excavation findings. Emergency Response personnel, Terry Whitehair and Fred Romeo arrived on location to assist in the assessment. We dug several test holes just south of the southern tank and found 4-6 of limestone gravel underlain and mixed with good clay that was unaffected by the release. It was decided at this time to have Enervest dig an interceptor trench south of the tank and flush the affected area with hot water to remediate the spill. I informed Enervest of the remedial plan, and they will have Scott Miller Contracting on location at 9:00am tomorrow (3/8/2016) to begin clean-up procedures. I will be on site in the morning to oversee operations.
