1006849024 | 2018-05-11 | Production Wells | No Violations | I conducted a status check inspection and follow-up to Incident # NR02201800001890 related to a flow line leak on 2/20/2018. The flow was replaced by Scott Miller Contracting and the well is currently in production. The stockpiled topsoil that was affected remains contained and covered north of the wellhead. I found a small pocket of oil (less than one (1) quart) just outside the southeast corner of the secondary containment dike. Two (2) underdrain siphon dams remain in place in the drainage ditch to the east and the ditch is dry and clean. The landowner and Enervest were given permission by Kurt Kollar (OEPA) to remediate (land farm) the topsoil with cow manure. I spoke with Kyle Ahbel for an update on the remediation plan and he told me that they were going to wait until summer to let the soil dry out and then remediate it with manure that the landowner is supplying. I asked him to put some absorbent pads down on the oil pocket and he said he would take care of it. I will continue to monitor the location through remediation. No violations. |
1519153614 | 2018-02-20 | Production Wells | No Violations | I was contacted by Emergency Response Coordinator Jason Blocker at 9:25am to inform me of Incident # NR02201800001890 concerning a flow line leak and the release of three (3) barrels of crude oil into a farm field and drainage ditch at this well location. I was on location at 10:13am with Enervest personnel and Scott Miller Contracting (SMC). The impacted area of impact in the field was 40 X 100 and approximately 500 of a drainage ditch to the east was lightly impacted. Pads had been placed along the course of the ditch and a soft boom was placed across the mouth of a culvert, 200 south of the impact, where drainage proceeds east towards Silver Creek. The ground was too wet to haul out the impacted soil and I instructed SMC to build a lined containment dike for stock piling the impacted material. SMC began scraping the area and Fred Romeo (Emergency Response) and Kurt Kollar (OEPA) arrived on location for assessment. OEPA took the lead and asked for four (4) underdrain siphon dams to be installed along the drainage ditch. Kurt spoke with the landowner, Mr. Brown, about keeping the topsoil and remediating in place. OEPA approved of land farming the soil with manure and sawdust under the oversight of DOGRM personnel. I remained on location and all the impacted soil was stockpiled and covered, the two (2) inch flow line was exposed and clamped, and the underdrain dams were installed. Enervest is not going to turn the well back on until the bad section of flow line is replaced. I will continue to monitor the location through remediation. |

The scraped area contains several small pockets of oil (less than one (1) cup) and the water collected in the bermed area upstream of the drainage ditch is clear. I inspected 500 yards of the drainage ditch that was impacted to the south and the remedial measures (underdrain dams) were working. Heavy rains over the prior week have flushed the impacted areas and small amounts of residual oil have collected in four areas upstream of the dams and can be recovered with absorbent pads. The absorbent pads placed along the course of the ditch last week can be collected. I inspected the soft boom in front of the culvert that drains to Silver Creek and found no evidence that oil has migrated to this point and water on both sides of the culvert is clear. I spoke with Kyle Ahbel (Enervest) and asked him to soak up the residual oil while the water level in the ditch is still high and remove the old pads from the ditch. The underdrain dams will remain until all the water is clear. I will check back later in the week. |
