| 0255624100 | 2014-05-12 | Production Wells | No Violations | I received a call from Joe Teppirt (Enervest) 12:15pm about an oil release from the stuffing box at this wellhead. I was on location at 1:30pm with Joe, Scott Miller Contracting, and J-Hawk Oilfield Services. Approximately 3-5 barrels of oil had leaked from the wellhead and flowed west in the ruts of the lease road and to the south into the weeds. The were several areas where oil had collected just south of the lease road in the edge of the woods. No drainage or waterways were impacted by the release. J-Hawk was using the vac-truck to collect the ponded oil and Miller Contracting was scraping out the weeds and impacted soil to the south. The soil will be removed to an approved landfill. I asked them to stake booms into the woods to contain any run off from the expected rain this evening. I will continue to monitor the clean up. |

The location has been cleaned up and a small amount of topsoil has been stockpiled and covered with a tarp for remediation. The wellhead is fenced and is equipped with an electric pumping unit. A gauge indicates 75psi on the tubing and there is a domestic hook-up on the flow line. ID is posted at the wellhead. The tank battery is diked and contains 2-210 barrel tanks, 1-100 barrel tank, and 2 vertical separators. The battery is diked and the posted IDs indicate that PN# 4236 produces to 1-210 barrel tank and this well and PN# 0363 produce to the other 210 barrel tank. I found no violations at this time. |
