0321832144 2013-08-20 UrbanProduction Wells No Violations I was on location to follow up on COMPLIANCE NOTICE #1363264163. This notice was issued under OAC 1501:9-9-05 E (5)b that prohibits storage of equipment used in the production of a well for longer than thirty (30) days. The notice ordered the removal of rods and tubing that were sitting on racks to the east and west of the wellhead and various other equipment that has been on location for over a year. My inspection found that none of the equipment in question has been removed. The rods are on the ground and have been pushed against the east side of the dismantled pumping unit. The tubing has been knocked off the racks and pushed to the west side of the pumping unit and presents a hazard. I called Bakerwells office in Westerville and left a message that someone needs to contact me as soon as possible to discuss the situation. This violation continues.
0470713793 2011-08-29 Production Wells No Violations I received a call from Mr. David Gale 2891 Waterloo road(330-612-3255) about the well in field that he doesnt think is producing. I would go out tomorrow and check on the site. This complaint continues.
1383929474 2013-11-06 UrbanProduction Wells No Violations Follow up to COMPLIANCE NOTICE #1363264163. I inspected the location and found that the rods and tubing have been removed from the location. The pumping unit has been removed and all extra equipment has been removed from the location. The weeds have been cut down and the lease road has been graded. A load of stone has been spread to the north side of the wellhead. The wellhead consists of 4.5 casing with 2 tubing exending 3 above grade and equipped with a balon valve that is closed. Bakerwell was issued a permit to plug back this Rose Run well to the Clinton Sandstone on 1/20/2012. The permit will expire on 1/19/2013. All compliance issues at this well have been properly addressed. This compliance notice is satisfied. No violations.
1710811374 2011-08-30 Production Wells No Violations I investigated the well site. I found the wellhead had 50psi on it. I called Rex Baker(330-276-2161) about this well. He stated that from January this year through April this year the well has made 500 mcf of gas. Rex stated that the pumping unit has a butane bottle that they us to produce the well when they cant get enough gas from the well to run it. Mr. David Gale stated that none of the houses are hooked up to the well because of the low gas. I passed that on to Rex. He has enough gas to start the unit again. I called David Gale to let him know about the well getting started up. This complaint is resolved.
1761051577 2010-10-25 Production Wells No Violations The Uniontown office received a call from David Gale (330-612-3255) who lives near the well and had concerns about its status. He said that the well has not produced since July and was worried that it was being abandoned. The jack is operated with a natural gas motor and apparently the fluid level is too high to pump off with the available gas supply. I spoke with Rex Baker (330-276-2161) who said that he would investigate the problem.
