0072385883 2013-10-22 Plug / Plug Back No Violations I was on location to check the plug from previous day. The surface plug had dropped four feet, was solid, and there was no sign of gas channeling. Chipco was rigging down and Jhawk was removing water from the lined pit. The top of the surface casing was cut below grade and the ID plate was tacked on top of the casing. This well has been properly plugged and abandoned. No violations.

Woody McDaniel was on location with a swabbing rig and preparing to rig up to the hole and swab the well to a steel tank. The 2 line was secure to the tank and all water is being contained. 0139690941 2013-07-21 Casing String Witnessed No Violations I was in location to witness the cementing of the 11.75 surface casing. PEP Drilling Rig #10 drilled a 17.5 hole to a depth of 100, tripped out of the hole, changed bits, and drilled a 14.75 hole from 100 to a depth of 344. They ran 7 joints of 11.75 casing and were unable to get the eighth joint past an obstruction in the bottom of the hole. The permit called for 306 (GL) (314 KB) of casing, 50 below the Big Injun. Leaving the eighth joint out put the casing at 295 (GL) (303 KB) and I allowed Enervest to continue with cementing operations with the casing set at this depth. PEP circulated on the hole for an hour until the cementers arrived. Universal Well Services rigged up, began circulating the hole, and mixed and pumped 53 barrels of Type I cement. The cement was displaced to a depth of 280 with 35 barrels of freshwater and the cement head was shut-in. 10-12 barrels of good cement returned to surface. No violations.
0250425125 2014-07-22 Final Restoration No Violations I was on location to inspect the Final Restoration status of this well site. The well was completed in the Rose Run Sandstone on 8/6/2013. After negative completion testing results, I approved plugging off of the drilling permit and the well was properly plugged and abandoned from 10/17/13-10/21/13. Preliminary Restoration was never necessary and all equipment used in completion testing was temporary and has been removed. No flow lines were ever buried and the lease road has been removed back to the Ruggles #2 Clinton well (PN#22148). The well was located in the middle of a bean/corn field and has been properly graded. No seeding is required as it will be replanted next year. Final Restoration is approved.
0272802925 2015-04-29 Final Restoration No Violations Status check of final restoration on this Rose Run well location. The well was completed on 8/6/2013 and stimulated on 8/12/2013. Flow back and production testing occurred for two (2) months and it was determined that the well was not economically viable. The well was plugged and abandoned between 10/17/2013 and 10/21/2013. No tanks or production equipment were ever permanently installed on this location. My inspection today found the location has been completely restored to an agricultural field and erosion controls have been removed. There is no equipment on location and a good seedbed has taken root in the field. The access road has been removed and pulled back to the lease road for the Ruggles #2 (PN# 22148). No landowner waiver (Form 5) is required for this location. Final Restoration is approved. No violations.

0558364997 2013-10-18 Plug / Plug Back No Violations I was on location to witness the plugging of this Rose Run well. Chipco found free pipe at a depth of 6340 and ripped the collar. One joint of 4.5 casing was pulled to 6298 to spot a 300 plug across the top of the Utica Shale (top of the Utica~6201). Petroset rigged up, began circulating the hole, and mixed and pumped 75 sacks of Class A cement with 2% gel. The cement was displaced with 93 barrels of water. Chipco pulled 36 joints of 11.6 lb/ft casing to a depth of 4786 to set a plug across the Clinton Sandstone (4625-4736). Petroset rigged up, broke circulation, then mixed and pumped 75 sacks of cement. The slurry was displaced with 69 barrels of water and the plug was set from 4786-4475. Petrset rigged down and left location. Chipco is going to pull 42 joints to 3022 and the remaining plugs will be set on 10/21/13. No violations.

I met with Bob Randall (consultant) and Jeff Foley (Enervest) to discuss drilling. Drilling had slowed dramatically during the previous day and Enervest made the decision to make a bit trip. The blow pit and liner were in good condition and the blow line was secured properly. No violations.

The annulus between the surface and production casing was opened prior to stimulation. The job was completed as designed and there were pressure deviations observed throughout the job. The pits have been solidified and closed and the site was being reclaimed. Kyle Ahbel (Enervest) was on location overseeing flow back operations. Fluids were being produced to a steel tank through a 2 steel line. The line was chained to the tank and all fluids were contained. No violations.

1375811805 2013-08-06 Drill / Deepen / Reopen No Violations I was on location today for the cementing of the production casing. The drilling contractor was PEP Drilling and they drilled a 7 7/8 hole to 7601. The casing used was 4.5 and was 11.6 lb casing and was set at 7576. Universal broke circulation and ran 150 BBLs of Class A cement. The plug landed and was held at 1200 psi for 5 minutes. The permit was in the dog house. Fire extinguishers were available in case of an emergency. No violations.
1378318696 2013-07-23 Drill / Deepen / Reopen No Violations I was on location at 10:30pm to verify casing integrity and test the blow out preventor (BOP) prior to drilling the cement plug. PEP Rig #10 placed 320lbs on the casing and I monitored the Pason gauge and inspected the BOP stack for 30 minutes. After thirty minutes, the gauge registered a drop of 5% and is within the maximum of 10% leak off. The BOP showed no signs of leaking during the test. Casing integrity has been verified and operations may continue. No violations
1385044641 2013-10-17 Plug / Plug Back No Violations I was on location to witness the setting of the bottom plug on this Rose Run well. This well was completed on 8/12/13, and, after testing the well, the company determined that the well was not commercially viable and they will plug the well off of the drilling permit. Enervest, Chipco, and Petroset were on location to conduct plugging operations. 2.375 tubing in the well was set at 7350 and I had Enervest run 3 more joints in the hole to 7440 to get below the bottom perforation at 7422. Petroset rigged up to the tubing, mixed and pumped 35 sacks cement, and displaced the slurry with 27.5 barrels of water at 1200psi. Water was circulated into a steel tank during the job. The plug was down at 11:59am and Chipco began tripping the tubing out of the hole. The 4.5 casing will be ripped in the morning and plugging operations will continue. No violations.
1385053442 2013-10-21 Plug / Plug Back No Violations I was on location to witness the setting of the final 3 plugs in this Rose Run well. Omega Cementing was on location, rigged up to the casing, established circulation, and mixed and pumped 75 sacks of cement slurry. The slurry was displaced with 43.4 barrels of water. Chipco pulled 45 joints to a depth of 1132 and omega set a 51 sack plug at the bottom of the surface casing. Chipco pulled 22 more joints and Omega set a 55 sack plug that circulated to surface. Chipco pulled the remaining 5 joints and Omega topped off the hole. The top plug held at the surface and the casing will be cut off below grade tomorrow and fitted with an ID plate. 151 joints of 11.6 lb/ft, 4.5 casing was recovered during the job. No violations.

The top of the Onondaga (Big Lime) was encountered at 2734. The rig was drilling 60/hour. The conductor and surface hole working pits were lined and had been pumped down. The blow pit was lined and in good condition. The steel blow line was staked and properly chained down. No violations.

Their plan is to connect the well to a separator and produce the gas to a sales line on the location and produce the fluids to a 500 barrel steel tank in order to determine if the well is commercially viable. Rods still need to be run into the tubing and should be done by the end of the week. No violations.

PEP drilled an 11 hole to a depth of 1045 and ran 23 joints of 8.625 casing to a depth of 1025 with a 10 landing joint. Universal rigged up, established circulation, and pumped 15 barrels of gel and 5 barrels of water spacer ahead of the slurry. They then mixed and pumped 79 barrels of Type I cement slurry, dropped the wiper plug, and displaced the plug and cement to 1003 with 64 barrels of freshwater. 25 barrels of good cement returned and held at the surface. No violations.
