0570474221 2014-03-25 Production Wells No Violations I was on location to obtain updated GPS coordinates for this well. During my inspection I found a tank battery with proper I.D. signage, two 210 bbl production tanks, brine vault and two vertical separators. The pumpjack well has a house gas connection, is nearby a residential house and is in good condition with a proper I.D. sign. The lease road is a residential driveway. There are no violations.
0833923170 2017-12-27 Production Wells No Violations I did a status check inspection for this well after receiving a phone call from Kyle Fink (Enervest Foreman) about a leak at the tank battery of approximately 40 barrels of fluid and oil. I arrived on location and met with Shawn Smith (Enervest Pumper) and he showed me the location of the leak. The leak is contained within the dike and is apprximately 170Ft. x 5Ft. The valve on the production tank for this well and well # 151-22103 had split in half releasing the contents of the tank. Joe Little excavating is on location conducting the clean-up. The well is equipped with a pumpjack and the production is shut-in. The tank battery consists of a steel production tank and is legibly identified. The dike is shared with the tank batteries for wells # 151-22132 & 22123.

1448986687 2015-12-01 Production Wells No Violations I was on location to follow up to a reported flowline leak for this well. The flowline has been repaired and the contaminated soil excavated. The location has been laid with lime, seeded and straw placed over the location for remediation. There are no violations at this time.
1535486475 2018-08-28 Production Wells No Violations I received a call from Fred Romeo (Oil & Gas Emergency Response) and Scott King (Emergency Response Supervisor) that a releaseof oil had occurred on SR-43 (Waynesburg Rd.). I contacted Tim McConaha (Enervest Superintendent) and Kyle Fink (Enrvest Foreman) to get the location on the spill. The following people/ companies were on location: Mike Burkhart (Enervest), Kyle Fink, B & E well treatment personnel, J Hawk Water services and Fred Romeo. The flowline was being worked on by B & E while doing a freshwater flush the flowline burst near SR-43. B & E personnel immediately ceased operations and secured the well. The line was repaired and pressured up to 100 psi and there were no leaks. I was advised by Mike Burkhart that ODOT instructed them on the road clean-up.
