0057446372 2019-01-10 Production Wells No Violations My inspection found the well is equipped with a pumpjack. The well has reported production for 2017. The tank battery consists of two steel production tanks, a horizontal separator, a vertical separator and a meter. There is a domestic gas connection at the tank battery. The tank battery is shared with wells #151-22042, 22148 & 22149. The well is legibly identified. No compliance issues are cited at this time.

0319406230 2015-06-30 Production Wells No Violations I was on location to follow up to the oil spill that occurred on 6-26-2015. The leak has been stopped and the contaminated soil excavated and disposed of at the Kimble landfill by Sutton Supply. Currently a siphon dam and a dug hole are catching any free product that is seeping out of the hillside or soil. I have requested an Enervest vacuum truck to be on location sucking to remove any residual free product in the siphon dam and the ditch. The hard boom is present still in the Hypocrite Run and is collecting any residual sheen on the water. The tank battery has been scraped of contaminated soil and disposed of. I have instructed Enervest to monitor the location and removal any free oil that may still be on location. I will follow up on the spill 7-2-2015 to check the progress.
0344918833 2004-06-15 Production Wells No Violations NO VIOLATIONS. GPS TB (1-shared 210 bbl oil).
0432368510 2015-06-26 Production Wells No Violations I was contacted at 9:45am by Troy Wells of Enervest stating that an oil spill had occurred at this wells tank battery. The 210bbl production tank for this well is shared with well #22148 and #22042. A load line runs from the tank to the street where oil haulers can pick up the oil load. The valve on the tank was left open and a leak formed inside the hillside on the load line. Approximately 4bbls of oil leaked from the load line before the flow of oil was stopped. The oil ran into a drainage ditch and eventually an oil sheen made it to Hypocrite Run. A hard boom was installed in the creek to stop the flow of oil in the stream. The location was tested for chlorides with Hach Quantabs and no detectable levels were found. Wade Balser of the OEPA was on scene at 2pm. The hillside near the leak in the flowline was excavated and the ditch dug out and disposed of by Sutton Supply at the Kimble landfill. Terry Whitehair of ODNR witnessed the steam cleaning of the ditch and installation of a siphon dam near the spill. The location was cleaned and prepared for a large rain storm that was coming through in the next 48hours. I will be on location 6-29-2015 to follow up on the spill.

The hillside is going to be dug back and any additional oil removed. I have requested an Enervest vacuum truck to be on location sucking to remove any residual free product in the siphon dam and the ditch. There is a leaking valve and brine flowline that I have requested be fixed by Sutton Supply this week.. The two (2) upright separators that were not in use have been steamed and removed from location by Sutton Supply. I have instructed Enervest to monitor the location and removal any free oil that may still be on location. I will follow up on the spill to check the progress.
