2421753 Primary Facility 2015-11-03 Plugging(Includes Plugged/Mined Through) In Compliance with Schedule Hydrocarbon back to cleaning/drilling w/ cable tools after being unable to regain circulation. Reached depth of ~570 feet w/ slow drilling & a lot of cavings, so stopped drilling & ran more 4 1/2" into hole. Got over 90 feet additional 4 1/2" casing in before it set down at around 513 feet, then drove a few more feet.

Chisler ran camera in well to look at condition of 7" casing. Casing had holes @ 167 feet, 232 feet and 233 feet, ratty collars @ 126 feet and 189 feet. Fluid level was at 233 feet. Decision was made to set cement basket on 4 1/2' casing @ 227 feet (above holes @ 232 & 233 feet in 7" casing). Hydrocarbon began running 367.45 feet of 4 1/2" casing.
2425216 Primary Facility 2015-11-17 Plugging(Includes Plugged/Mined Through) In Compliance with Schedule Hydrocarbon completed cementing 4 1/2" casing vent in place inside 7" casing after setting bottom plug. Pulled all 28 feet of 10" casing sleeve that they had run earlier to stabilize caving around backside of 7". Grouted backside of 7" casing, then filled larger hole near surface w/ gravel. Installed two 2" PVC vents; one on 4 1/2" casing vent and the other extends into gravel bed. Remoting vent pipes away from house and driveway ~75 feet. Will be pouring a concrete pad over vents and surrounding area in preparation to restore driveway.
