2423421 Primary Facility 2015-11-09 Routine/Complete Inspection No Adverse Event or Action Reported Received call from Larry Birch of Columbia Gas, reporting open, gassing abandoned well behind the Paradise Island Bowl. Met Rich Freese on location after he had met w/ Larry. Well is open 8" casing to surface, filled to about a foot from surface w/ mud, gravel and debris, with standing water. Gas is freely bubbling through the water. Gas meter readings (62-S): 5% methane, 100% LEL inside casing. No gas readings in air around well, but windy during tests. Went in building, took numerous readings along back wall beside well: All were 0% methane, 0% LEL.
2423435 Primary Facility 2015-11-09 Routine/Complete Inspection No Adverse Event or Action Reported Well is open 8" casing to surface, two feet from back wall of building, filled to about a foot from surface w/ mud, gravel and debris, with standing water. Gas is freely bubbling through the water. Gas meter readings (62-S): 5% methane, 100% LEL inside casing. No gas readings in air around well, but windy during tests. Went in building, took numerous readings along back wall beside well: All were 0% methane, 0% LEL.
2424230 Primary Facility 2015-11-10 Routine/Partial Inspection No Adverse Event or Action Reported Took gas readings from ~15 holes in yard around well and along the back of the bowling alley. All had 0% LEL. Was raining at the time of tests.
2425217 Primary Facility 2015-11-17 Routine/Partial Inspection No Adverse Event or Action Reported Visited site to inspect for changes in gas emissions since 11/10/15 inspection - none. Robert Morris is planning on extending a sidewalk from their adjoining sports facility across the back of the bowling alley yard and down to the Ohio River as access for womens rowing. Staked path of proposed sidewalk passes over area near well where a plugging rig would be located during plugging operation. Discussed alternate options by phone with engineers handling the project to avoid damaging concrete walk by postponing installation until after plugging.
2425731 Primary Facility 2015-11-18 Routine/Partial Inspection No Adverse Event or Action Reported Visited site w/ Dave McDermott (DEP). Group of indiviuals from neighboring Robert Morris Athletics Dept. (Donald Smith and others) and engineers were on site discussing proposed sidewalk to be constructed across back of bowling alley property down to Ohio River to provide the womens rowing team access from their sports facility to the river. The proposed path is within the area that would be occupied by a plugging rig, so I recommended a temporary path be installed until the plugging can be completed.
2427523 Primary Facility 2015-11-24 Routine/Partial Inspection No Adverse Event or Action Reported
2428285 Primary Facility 2015-11-30 Routine/Partial Inspection No Adverse Event or Action Reported Visited site to check well for changes in quantity of gas vented, none noted.
2432671 Primary Facility 2015-12-14 Routine/Partial Inspection No Adverse Event or Action Reported Checked well for any changes in gas venting; appears the same as prior inspections. Met w/ Mascellino Contracting, the contractor working on the Robert Morris sidewalk to be installing across Paradise Beach #1 wellsite. It appears current layout may not conflict with future plugging operations. Construction will likely begin this week. Buckeye Pipeline met earlier w/ Mascellino to discuss construction near and over Buckeye's line and confirmed depth of pipeline @ 3 1/2 feet.
2434274 Primary Facility 2015-11-09 Plugging(Includes Plugged/Mined Through) No Violations Noted I MET WITH A COLUMBIA GAS REP AT THIS WELL SITE. THE WELL IS LOCATED 2' FROM THE FOUNDATION OF A BOWLING ALLEY. THHERE IS A KITCHEN LOCATED ON THE OTHER SIDE OF THE WALL. THE WELL IS BUBBLING LIKE RAPIDLY BOILING WATER. WITH A MSA 62-S THE GAS WAS 5% AND THE LEL WAS 100%. ALTHOUGH GAS WAS NOT DETECTED AROUND THE WELL CASING IT WAS NOT POSSIBLE TO SAMPLE UNDER THE BUILDING. THE GAS COMPANY EXPRESSED THIER CONCERNS AND IS MONITORING GAS LEVELS INSIDE AND OUTSIDE ON A WEEKLY BASIS.THE WELL IS FULL OF WATER, SAND, ANDGRAVEL WHICH MAKES IT ODD THAT THE GAS CAN MAKE IT TO THE SURFACE. I BELIEVE THESE CONCERNS SHOULD BE REASON TO CONCIDER THIS WELL AN EMERGENCY.
2434576 Primary Facility 2015-12-21 Routine/Partial Inspection No Adverse Event or Action Reported No change in gas leakage, but water level in the casing appears to be ~ one inch lower. Mascellino Contracting has begun excavating for Robert Morris sidewalk installation.
2436204 Primary Facility 2015-12-28 Routine/Partial Inspection No Adverse Event or Action Reported Measured exact OD of casing (8 3/4") so Appalachian Well Services could acquire materials to install vent system this week. Excavation continues on the Robert Morris walkway being installed adjacent to wellhead.

2440270 Primary Facility 2016-01-13 Routine/Partial Inspection No Adverse Event or Action Reported Checked vent, couldn't tell if venting. Temperature was 7 degrees and I was concerned water in casing was frozen. Snow melted off of casing vent cap, but surrounded by recent snow, indicating warm enough to not freeze.
