2009565 Primary Facility 2011-10-06 Routine/Complete Inspection Outstanding Violations - No Viols Req'd Drill pit had been regraded. The other issues outlined in the 08/18/11 Inspection and 08/26/11 NOV had not been addressed as of the time of the inspection. At this time the Department is requesting that the operator forward documentation showing that any pollutional materials, i.e. the observed petroleum product in the drill pit during the 08/18/11 inspection, were properly disposed of and any drill cuttings contained in the drill pit were properly disposed of. This information should be forwarded no later than November 2, 2011. This information should be sent to the attention of Water Quality Specialist Greg Higgins at the above listed address.
2026471 Primary Facility 2011-12-14 Follow-up Inspection Outstanding Violations - No Viols Req'd Operator had undertaken activity to address the issues outlined in an inspection and NOV from August of 2011. The drill pit area had been regraded, the majority of the oil stained media had been removed from the well pad save for the area immediately underneath the service rig onsite, and the operator had installed silt fencing as a perimeter erosion control. The operator should forward soil sample results showing that all of the oil contaminated media had been removed upon completion of the excavation activity.
2038991 Primary Facility 2012-02-01 Follow-up Inspection Outstanding Violations - No Viols Req'd Found the site to be in the same condition as it was on 12/13/1 inspection. The drill pit area had been regraded, the majority of the oil stained media had been removed from the well pad save for the area immediately underneath the service rig onsite, and the operator had installed silt fencing as a perimeter erosion control. The operator should forward soil sample results showing that all of the oil contaminated media had been removed upon completion of the excavation activity. The Department recommends that the operator complete the activities requiring the rig onsite in addition to completing all neccesary activities to resolve the outstanding issues related to the NOV from 08/26/11.
2064357 Primary Facility 2012-05-02 Compliance Evaluation Outstanding Violations - No Viols Req'd Site was found in similair condition as the most recent inspection. Drill rig still located onsite. According to clean-up contractor McCutcheon excavation and sampling was undertaken and no evidence confirming the petroleum products buried in the excavated drill pit was found. The operator had taken minimal action to remove the herbaceous layer which absorbed oil which was spilled during well work-over.
2141529 Primary Facility 2013-02-21 Follow-up Inspection Outstanding Violations - No Viols Req'd Well appeared to be equipped for production. Site was in the same general condition as it was in the 05/02/12 inspection. Drill rig still located onsite and the affected area as a result of the crude oil spill had not been remediated. The site had still not been restored.
2685349 Primary Facility 2018-01-17 Compliance Evaluation Outstanding Violations - Viols Req'd This well is tagged, in production and is leaking gas to the atmosphere in a manner which presents a hazard to the public. There is a 1" brass ball valve located on top of the well drip tank which has a split in the side of the valve body. This split valve is not leaking yet and is not the source of the undetermined leaking gas but this valve will need replaced before it begins to leak. There is a service rig, rig doghouse, string of production tubing sitting on a pipe skid, a D4 Cat Dozer and various other equipment sitting on this well location where it has been for a number of years now. The operator has failed to submit either the annual required mechanical integrity report for this well or the annual required production report for this well.
2734829 Primary Facility 2018-05-29 Compliance Evaluation Outstanding Violations - Viols Req'd This well is tagged and in production. This is a follow up compliance evaluation inspection. Since my last inspection, the gas leak which I could smell but could not find due to snow cover has been stopped. The brass drip valve which had a small split in the side of the valve body which was not leaking yet has been replaced with a new valve. The service rig is still standing over the well and the rig doghouse, various equipment, a D4 Dozer and a string of 4 1/2" well casing along with various other debris which was all to be removed still sits on this well pad. My inspection finding for this well is that it is in outstanding, continuing violation status.
2820117 Primary Facility 2018-12-28 Compliance Evaluation Violation(s) & Outstanding Violations Pursuant to an order issued to remove equipment unnecessary for producing a well within 9 months of the completion of drilling a well, this well remains in continuing and outstanding violation as a service rig, rig doghouse, string of production pipe, D4 Cat Dozer and various other equipment is still sitting on this well site years after the service work on this well was completed.
2831331 Primary Facility 2019-01-03 Follow-up Inspection Violation(s) & Outstanding Violations FUI to inspection record #1998584 where violations were issued for constructing an improperly lined pit, for failure to achieve permanent stabilization, failure to restore site, failure to notify DEP of a pollution incident, and failure to properly store, transport, process, or dispose of a residual waste. The violation for an improperly lined pad was corrected on 12 – 14 –11 (See inspection record #2026471).

An Order (Enforcement #362085) was issued against this well on 3-5-18. At time of inspection well appears equipped for production. Well tag is present. Brine tank appears structurally sound. At time of inspection, service rig noted in inspection record #2064357 remains on site (photo #1). There is also production tubing, a rig doghouse, a dozer, and various tools and containers on site (photo #2–4).
