E&S controls had been implemented as outlined in the E&S plan and no accelerated erosion & sedimentation was observed in relation to the access road or well-site. Below grade drill pit located onsite and it had more than the required two feet of lined freeboard and the pit liner appeared to be intact.

2198504 Primary Facility 2013-08-22 Follow-up Inspection Outstanding Violations - No Viols Req'd Well appeared to be equipped for production at the time of the inspection. It was noted that a drill rig and several other well development components were located onsite at the time of the inspection. The site had not been properly restored. The site had not been temporarily or permenantly stabilized, however there was no observed accelerated erosion or sedimentation in relation to the site. It did not appear that the perimeter erosion controls had been properly maintained. There was minor erosion related to the access road which required maintenance to insure its continue passability. There was no well tag observed on the well apparatus located onsite. It was noted that the site was in the same condition as it was during the 07/10/13 inspection and the requested docummentation had not been forwarded to the Department from the operator.
2685340 Primary Facility 2018-01-17 Compliance Evaluation Outstanding Violations - Viols Req'd This well is tagged and in production. The operator is in outstanding violation status as a service rig has been left sitting up over the well for a number of years and is now leaning in an unstable manner, and the rig doghouse, pipe skids, multiple tanks, 3 compressors and various other equipment has also been left sitting on this site for years. The operator has also failed to submit either the required annual mechanical integrity report or the required annual well production report.
2734812 Primary Facility 2018-05-29 Compliance Evaluation Outstanding Violations - Viols Req'd This well is tagged and in production. This inspection is a follow up compliance evaluation inspection. Nothing has changed on this well pad since my last inspection. The service rig is still standing over the well. The rig doghouse, compressors, fluid collection/fuel tanks, various pieces of equipment and other debris still sit on this well pad. It is my finding that this well is in outstanding, continuing violation status.
2820091 Primary Facility 2018-12-28 Compliance Evaluation Violation(s) & Outstanding Violations Pursuant to an order to remove all equipment not necessary for the production of the well within 9 months of the completion of drilling the well, this well remains in continuing and outstanding violation as a service rig, rig doghouse, pipe rack with pipe, compressor, two fuel tanks, two generators and various other equipment remains on this well pad years after the completion of working on the well.
2831474 Primary Facility 2019-01-23 Follow-up Inspection Outstanding Violations - Viols Req'd FUI to inspection record #2184837 where a violation was issued for failure to achieve permanent stabilization (25 Pa Code §102.22).

An Order (Enforcement #362085) was issued against this well on 3-5-18. At this time operator has not met all conditions of the Order. During this inspection access road and site are snow-covered. At time of inspection well appears equipped for production. Well tag present. Service rig noted in inspection record #2198504 remains on site (photo #1). There is also other well development equipment on site (photo #2). Temporary BMP's are installed on the southern side of the well pad (photo #3). Violation issued for failure to achieve permanent stabilization remains outstanding. A follow up inspection will be conducted at a later date. Additional violations issued by DEP OGI remain outstanding.

2855369 Primary Facility 2019-03-19 Compliance Evaluation Outstanding Violations - Viols Req'd This is a compliance evaluation inspection to check and see if the service rig and equipment not necessary for the production of this well have been removed yet as per the order issued to do so. The service rig, doghouse, fuel tanks, compressor and other equipment are still sitting on this well location. See attached photos.
