Inspection Data

Inspections Performed

Inspection ID Inspection Category Inspection Date Inspection Type Inspection Result Inspection Comments
1727707 Primary Facility 2008-08-05 Routine/Complete Inspection No Violations Noted well staked - site not made/waiver ok as revised
1733439 Primary Facility 2008-09-05 Drilling/Alteration No Violations Noted Well spudded 9/4/08. Rig on site @ inspection. Pit/freeboard/E&S controls Ok.
1734531 Primary Facility 2008-08-26 Routine/Complete Inspection No Violations Noted Site constructed, drilling had not begun. Access road heavily rocked, and rocked ditch installed beside road. Culvert placed immediately before the site had temporary BMP's at the outlet. The operator should monitor the well-site & access road for accelerated E&S until final restoration is completed, specifically the operator should monitor the toe of fill slope on the NE side of the site for runoff & subsequent accelerated erosion & sedimentation.
2115266 Primary Facility 2012-11-13 Routine/Complete Inspection No Violations Noted Drilling had been completed, well had recently been stimulated. Site had been regraded, seeded and mulched. The vegetation had not begun to take hold. The Department recommends that the operator continue to monitor the site to insure that 70% uniform vegetative cover is achieved and to insure that the collection ditch and rock lined outlet continue to function properly.
