1897362 Primary Facility 2010-07-09 Routine/Complete Inspection No Violations Noted Drilling was about to commence at the time of the inspection. Below grade pit located onsite. Pit was relatively empty. Pit liner appeared to be intact and no deficiencies (i.e. holes, tears, bad seams) were observed. E&S control measures had been implemented as outlined in the E&S plan. A collection ditch implemented on the side closest to the cut slope should be extended and stabilized so as to direct the runoff past the silt fencing to the well vegetated area. Spoke with operator representatives Doug Miller and Todd Kunselman regarding this minor issue.
1900008 Primary Facility 2010-07-22 Routine/Complete Inspection No Violations Noted Drilling was ongoing at the time of the inspection. Below grade pit onsite and it had more than the required two feet of lined freeboard. Observed no accelerated erosion & sedimentation in relation to the access road or well-site. All erosion control measures appeared to be installed and functioning properly.

Pit had more than the required two feet of lined freeboard. Site had not been reclaimed. Observed no accelerated erosion & sedimentation issues in relation to the access road or well-site.

Brine tanks located onsite and they appeared to be in sound condition. Noted that the drill pit was still located onsite. Pit contained cuttings, fluids and snow. Noted a hose running over the ground from the production apparatus to the drill pit. Pit had more than the required two feet of lined freeboard. Site had not been reclaimed. Observed no accelerated erosion & sedimentation issues in relation to the access road or well-site. The Department recommends that the operator begin site regarding and restoration activities as soon as possible due to the expiration of the nine month restoration time frame on 04/27/11.
2044733 Primary Facility 2012-02-23 Site Restoration No Violations Noted Drilling was completed on 07/27/10. Well appeared to be equipped for production. Brine tanks located onsite and they appeared to be in sound condition. There was no observed accelerated erosion or sedimentation in relation to the access road or well-site. Site had been properly reclaimed and the vegetative cover met the 70% uniform coverage criteria.
2122172 Primary Facility 2012-12-10 Routine/Complete Inspection No Violations Noted Well appeared to be equipped for production. Brine tanks located onsite and they appeared to be in sound condition. There was no observed accelerated erosion or sedimentation in relation to the site or access road.
2558961 Primary Facility 2017-02-02 Routine/Complete Inspection No Violations Noted Well tagged-in production. Annulus was open. Well head casing-production set equipment-2 grounded 100 bbl fiberglass tanks looked ok.
