At time of inspection well, located in an ag field, appears to be plugged. No E and S issues observed. No temporary BMP's on site. Site appears to be returned to near original contours. No monument present. According to operator, a plate is buried below plow depth. This Restoration Report is approved.

2829157 Primary Facility 2019-01-17 Routine/Complete Inspection No Violations Noted On 1/17/19 at 1132 hrs., I conducted a routine inspection of Snyder Brothers, Inc.'s Robert E Patrick Jr 12M-197 well (API 005-30856) in. Mahoning Twp., Armstrong Co. This well has been plugged and the site has been restored for agricultural use. The well status is still listed as "Active" in eFacts. I requested that NWRO Oil & Gas staff change the well's status on 1/18/19. There are no violations noted for this inspection.
