Operator is waiting to finish the injection test on the 4H well before performing an injection test on the 1H well. All equipment on site looked ok.

The operator is requesting a six month extension from July 2018 to January 2019 due to lack of essential fuel, equipment, or labor.

The last well reported to be drilled on the pad is the 1H (005-31273); drilling was complete on this well 10/14/2017, making the restoration deadline 7/14/2018. After review, a restoration extension is being granted until January 14, 2019.

2760916 Primary Facility 2018-08-07 Routine/Complete Inspection No Violations Noted This well has now been fracked and is awaiting the arrival of the snubbing unit to drill out the plugs. All equipment on site looks ok.
2783813 Primary Facility 2018-10-03 Administrative/File Review No Violations Noted ADMIN inspection due to operator submittal of a Well Site Restoration Extension Request. Operator states extension is requested due to weather conditions and limited contractor availability.

This extension request is approved. Well Site Restoration date is now June 2019.
