1826131 Primary Facility 2009-09-01 Drilling/Alteration No Violations Noted At time of inspection, I met with Company Man Kenny Lloyd. The drilling rig on site is Nomac Rig 117. Mr. Lloyd indicated that drilling depth was at 10,900'. TD is anticipated to be 11,400'. Once TD is achieved, 5-1/2" production casing will be installed and cemented to surface. The drillers log was on site and kept up to date. Freeboard in pit was approximately 3¿ and the liner appeared to be in good condition. The site appeared clean and free of spilled drilling fluids.

1978674 Primary Facility 2011-06-02 Complaint Inspection No Violations Noted Well servicing operations were in progress at the site. A complaint was filed to the Department regarding potential pollution at the site. Conducted an inspection of the site with Sean VanFleet, Solid Waste Specialist in the NorthCentral Region. The site is utilized as a water processing facility for flowback water. There are 8 raw water tanks containing flowback water from fracing operations from various Chesapeake well pads. The flowback water goes through a filtering process consisting of 4 20 micron filtering units, with 6 to 8 filter socks in each unit. The filtering process separates the solids and the liquids in the flowback water. The liquid portion of the water goes into to 12 filtered water tanks to be reused for fracing operations at Chesapeake sites. The filter socks are sent to a landfill for disposal.

1990291 Primary Facility 2011-07-14 Follow-up Inspection Violation(s) Noted Department records indicate that Chesapeake has failed to restore the site within nine (9) months after completion of drilling, since no drilling has occurred at this site since October 10, 2009 and associated stimulation of the Hershberger 2H and 5H wells on the pad was completed on March 5, 2010. This constitutes a violation of Section 206(c) of the Oil and Gas Act, 58 P.S. §601.206(c). The Department has revealed the evidence of fluids being treated and stored at the Hershberger 2H, 5H well pad since no wells have been drilled since October 10, 2009. The site operations constitute an unpermitted residual waste processing facility. The violations in reference to this are Section 301 of the PA Solid Waste Management Act, 35 PS § 6018.301 and Section 6018.610, item numbers 2,4 and 9 of the PA solid Waste Management Act, 35 P.S. § 6018.610.
