2208418 Primary Facility 2013-09-26 Incident- Response to Accident or Event No Violations Noted Arrived at the site at 11:20 in response to a (25) gallon spill of an unknown material. Chesapeake reported the spill incident to the Department at 6:42 PM on 9/25/13. The Marcellus incident report indicates the spill occurred as Chesapeake was getting ready to move frac equipment. They were moving a dumpster not on containment and in the process of cleaning it out. The report alleges a liquid leaked out of the container and is most likely water. The liquid impacted the soil around the dumpster and Chesapeake already scraped the soil and staged it on containment for removal.

2633729 Primary Facility 2017-09-07 Site Restoration No Violations Noted Two Wells in production phase. Two each sand and water separators. Two production water storage tanks. No leaks or corrosion noted on production equipment. Site stable. Site recommended for notice of termination approval. No violations noted.
2719170 Primary Facility 2018-04-17 Routine/Complete Inspection No Violations Noted Arrived at 12:30. Weather: cold and windy. Two producing wells onsite. Two sand separators, 2 GPUs, two produced water tanks. No violations were observed.
2855917 Primary Facility 2019-03-18 Routine/Complete Inspection No Violations Noted Arrived onsite for a routine inspection at 11:07; the weather was ~35°F, calm, and sunny. A well tender was leaving the pad as I arrived.

There were 2 properly tagged Marcellus wells staged for production. There was some ice and water in the cellars; no bubbling was observed. There were 2 each GPUs, sand separators, and produced water tanks also onsite.

There was an infiltration area installed to the west of the pad as depicted in PCSM plans. The majority of the area was greater than 70% vegetative cover or rock-lined and appeared stable. The southeast corner of the cut slope showed large amounts of erosion (Photos #3-5). An orange safety fence was in place above the eroding bank.

It is recommended that the southeast corner of the pad be stabilized in order to prevent more slope damage.

No violations noted at this time.
