2500916 Primary Facility 2016-07-08 Site Restoration No Violations Noted On July 8, 2016, at 1220 hrs.,the Department conducted a site restoration inspection at the Chesapeake Appalachia LLC, Storms Location in Tuscarora Township, Bradford County. Andrew Hobbins, Water Quality Specialist, represented the Department. Nobody was present during the inspection on behalf of Chesapeake. A Notice of Termination was received by the Department on 6/28/2016.

Generally the well pad location was determined to meet the requirements of 25 Pa Code 102.22(a)(2)(i) Permanent Stabilization. The well pad location was observed to be vegetated greater than 70% and all erosion and sedimentation BMPs were observed to be removed. Both PCSM infiltration basins were in place and functioning as designed. No trash or non-essential equipment was observed on site.

Based on the conditions observed during the inspection, the NOT is recommended for approval.

The Department left the site at 1300 hrs. No violations were observed during the inspection.

2674525 Primary Facility 2017-12-18 Routine/Complete Inspection No Violations Noted On December 18, 2017, the Department conducted a routine inspection at the Chesapeake Appalachia LLC, Storms Location in Tuscarora Township, Bradford County. The Department was represented by Andrew Hobbins. Nobody was present for Chesapeake.

The site sign, E & S plan and site inspection logs were on site and available for review. The site was well vegetated and and no erosion issues were observed. The well pad has not been reduced in size, but PCSM has been addressed at the site.

Two wells were in production during the inspection. The wells were labeled and no bubbling was observed in the well cellars. Two production tanks and two GPUs were also on site. Puddles on the pad were observed to be frozen so no conductivity readings were taken. No staining was observed on the well pad surface.

No violations were observed during the inspection.

2823009 Primary Facility 2019-01-04 Routine/Complete Inspection No Violations Noted Arrived onsite for a routine inspection at 09:36; the weather was ~33°F, calm, and clear.

There were 2 properly tagged Marcellus wells staged for production. The well cellars were filled with water; no bubbling was observed. There were 2 each GPUs, produced water tanks, compressors, and sand separators onsite and in secondary containment.

There were some frozen puddles on the pad surface. Overall, the site was greater than 70% vegetative cover. No E&S issues were noted and PCSM was constructed and appeared to be functioning as intended. The pad had not been reduced in size and appeared to match the "Production and Operations" plan onsite.

The Department recommends inspecting and maintaining or replacing production and storage equipment as needed.

No violations noted during this inspection.
