2586493 Primary Facility 2017-04-24 Routine/Complete Inspection No Violations Noted On April 24, 2017, at 1130 hrs, the Department conducted a routine inspection at the Chesapeake Appalachia LLC, Mowry 2 Location in Tuscarora Township, Bradford County. Andrew Hobbins, Water Quality Specialist, represented the Department. Nobody was present for Chesapeake.

The E&S plan, site sign and weekly inspection logs were on site and available for review. Two PCSM infiltration areas were observed to be installed at the Mowry 2 Location. The site was well vegetated and no erosion issues were observed.

Two wells were observed on site and are currently inactive. Both wells were granted inactive status on 3/24/2016. There was no bubbling observed in the well cellar. The wells were not labeled with the well permit number during the inspection. The Department recommends labeling the wells with the appropriate well permit number. Two production tanks and two GPUs were also observed on site. The production tanks were in secondary containment. The containment was observed to be in good condition d

2744537 Primary Facility 2018-06-22 Incident- Response to Accident or Event Violation(s) Noted About 18 gallons of flowback was discovered during the removal of containment. No visible cuts,tears or seam failures were discovered in the polyurethane plastic containment. The area was used for truck parking and thur-traffic during the frack. Excavations will begin once an excavator and Leidos can be on location (first cleanup to begin on Monday 6/25/2018). No sample collected at this time.

Update 6/26/2018: Location was excavated on 6/25/2018. Leidos collected two samples when they believed the depth of the excavation was reached. The material removed is believed to be around 1 1/4 tri-axle dump truck loads. Area was backfilled once Leidos completed their sampling. Leidos will deliver a full report to the Department once their investigation is finalized. The spilled amount remains at 18 gallons.

The Department believes the clean-up to be complete at this time and will address site conditions again once a report is delivered by Leidos. Violations noted for spilled material.

2801579 Primary Facility 2018-11-08 Routine/Complete Inspection No Violations Noted Chesapeake Appalachia submitted a Well Site Restoration Report (WSRR) for the Mowry 2 pad. The WSRR was received by the Department on 10/31/18.

Arrived onsite at 11:23, the weather was calm and ~45°F. There were several puddles on the pad surface. Multiple locations were screened for conductivity and all measurements were between 150 and 325 µS/cm. An incident was reported in inspection #2744537. The conductivity of water in the excavated and backfilled area mentioned in that report was 250 µS/cm.

There were 2 properly tagged Marcellus wells staged for production. The tags on the wells were beginning to peel and were missing some information.There were 2 GPUs and sand separators and 1 double-walled fiberglass produced water tank onsite.

The pad appeared to remain at is original size and was greater than 70% vegetative cover. No E&S issues were observed. According to Department records an NOT was acknowledged for this site on 10/13/15.
