2249035 Primary Facility 2014-02-25 Routine/Complete Inspection No Violations Noted In a relatively close proximity to the access road of the site, there is a (500) barrel Great Plains frac tank on secondary containment with a pump system. Mr. Harer stated that the tank contains mud stored on the Storms Pad that potentially may have been used in the event anything happened to one of the wells during well stimulation procedures. Apparently there was concern the casing may give out on the well. There is about (280) barrels in the tank according to Mr. Harer. The pump aids in circulating the mud to prevent it from freezing. The mud has been circulating in the tank for a few weeks at this location and the pump runs (24) hours a day. There is also a heating system in place in the secondary containment system to prevent the mud from freezing. Mr. Harer also mentioned well stimulation procedures were shut down on the Storms Pad due to the mud freezing.

2589644 Primary Facility 2017-04-28 Routine/Complete Inspection No Violations Noted On April 28, 2017, at 1225 hrs., the Department conducted routine inspection at the Chesapeake Appalachia LLC, Duffield Location in Tuscarora Township, Bradford County. The Department was represented by Andrew Hobbins, Water Quality Specialist. Nobody was present for Chesapeake.

The site sign, E&S plan and weekly inspection logs were on site and available for review. The site was well vegetated and no erosion issues were observed. PCSM was addressed at the site. A Notice of Termination was approved by the Department on 10/13/2015.

There were two wells in production during the inspection. Both wells were labeled and no bubbling was observed in the well cellar. One Production tank and two GPUs were observed on site. The production tank did not have a containment structure surrounding it. The Department contacted Eric Hottenstein of Chesapeake. Mr. Hottenstein stated that the tank is a new fiberglass double walled tank that Chesapeake is testing. A containment structure is no necessary as the o

2697802 Primary Facility 2018-02-20 Routine/Complete Inspection No Violations Noted On February 20, 2018, the Department conducted a routine inspection at thr Chesapeake Appalachia LLC, Duffield Location in Tuscarora Township, Bradford County. Andrew Hobbins represented the Department. Nobody was present for Chesapeake. The Department did speak with the landowner who arrived at the site shortly after the Department.

The site sign, E & S plan and weekly inspection logs were on site and available for review. The site was well vegetated. An area on the fill slope needed to be repaired and temporarily stabilized with mulch. The well pad has not been reduced in size, but PCSM has been addressed.

Two wells were in production during the inspection. The wells were tagged and no bubbling was observed in the well cellar. One production tank and two GPUs were also on site. The production tank was observed to be a fiberglass, double walled tank. There was no staining on the surface of the pad. Numerous puddles were observed on the pad surface, but all were low in specific conductivit

2822842 Primary Facility 2019-01-04 Routine/Complete Inspection No Violations Noted Arrived onsite for a routine inspection at 10:35; the weather was ~35°F and calm.

There were 2 properly tagged Marcellus wells staged for production. The well cellars were filled with water; no bubbling was observed. There were 2 GPUs and 2 sand separators onsite and in secondary containment. A single fiberglass produced water tank was also onsite.

There were some areas of ice on the pad surface. The area was greater than 70% vegetative cover and no E&S issues were observed. The infiltration area along the southeast edge of the pad was retaining some water and ice (Photo #1). This could be due to the frequent rainfall and colder temperatures.

No violations were noted at the time of inspection.
