2618374 Primary Facility 2017-07-21 Routine/Complete Inspection No Violations Noted On July 21, 2017, the Department conducted a routine inspection at the Chesapeake Appalachia, LLC, Lionetti Location in Tuscarora Township, Bradford County. Andrew Hobbins, Water Quality Specialist, represented the Department. Nobody was present for Chesapeake.

The site sign, E & S plan and weekly inspection logs were on site and available for review. The slopes of the well pad were well vegetated. No erosion or sedimentation issues were observed. PCSM has been addressed at the site.

Three wells were in production during the inspection. The wells were labeled and no bubbling was observed in the well cellars. One double walled fiberglass production tank was observed on site. The second wall of the tank acts as secondary containment. A leak detection system within the double walled tank will automatically shut in the wells if a leak is detected in the production tank. The tank was in good condition during the inspection. Puddles within the secondary containment were low in specific conductivit

2809557 Primary Facility 2018-12-03 Routine/Complete Inspection No Violations Noted Arrived onsite for a routine inspection at 11:36; the weather was ~45°F and calm. The 103H well was stimulated in October 2018 so some materials remained onsite (Photo #2). There were a number of puddles on the pad surface. Multiple locations were screened for conductivity and all results were below 500 µS/cm.

There were 4 properly tagged wells staged for production. The well cellars were filled with water; no bubbling was observed. There were 2 GPUs and 5 sand separators onsite and in secondary containment. One fiberglass produced water tank was also onsite.

Some areas on this inside of the pad containment berm showed wear possibly due to the recent work onsite (Photo #1). No E&S issues were noted off of the pad area, although some restoration actives will probably be needed when weather permits. Some of the water deflectors on the access road have been flattened out and may need to be repaired or replaced to maintain their effectiveness (Photos #3, 4, 5).
