2311039 | Primary Facility | 2014-10-09 | Routine/Partial Inspection | Outstanding Violations - No Viols Req'd | 10/9/14 - 13:00 field readings indicate elevated conductivity near gas wells and in front of production units. Sampled at 5 locations. Previously on 9/25/14 obtained elevated field conductivity readings for these puddles ranged from 3mS/cm to 9 mS/cm. |
2342200 | Primary Facility | 2015-02-04 | Administrative/File Review | No Violations Noted | On 2/4/15 the Department reviewed the Site Characterization Report for remediation activities for the high conductivity fluid puddles Chesapeake identified on the Dan Ellis BRA well pad and reported to the Department on February 12, 2014, and the subsequent Department inspection conducted on March 25, 2014. The report was prepared by Leidos Engineering and received by the Department on November 21, 2014. The review was conducted in accordance with the Alternative Remediation provision of the Department policy document 800-5000-001 Addressing Spills and Releases at Oil and Gas Well Sites or Access Roads. No technical deficiencies were noted. Violations cited in inspection report 2280970 are considered resolved. |

2730044 | Primary Facility | 2018-05-15 | Routine/Complete Inspection | No Violations Noted | Arrived at 1315. Weather: windy, unsettled, threat of storm. There were three producing welld at this location. No violations reserved at the time of this inspection. |
2817074 | Primary Facility | 2018-12-14 | Routine/Complete Inspection | No Violations Noted | Routine inspection to assess overall site conditions. At the time of the inspection, a work crew was placing rip rap along a portion of the pad side slope on the east side of the pad. The topsoil stockpile had been reseeded recently as well.
Three wells are staged for production. Three GPUs and sand separators are present without secondary containment and appear to be in good condition. Two steel produced water tanks are present and sit within steel containment with spray-in liner. The tanks and containment are in good condition overall. The berms on the east side of the pad appear to be functioning as designed. They do not appear to be retaining water. A stream crossing is present along the access road. Two 96" corrugated metal pipes are staggered. Upstream the pipes are level with the stream bed. Downstream there is a drop immediately following the pipes and a tub is present here. The stream bed is eroded down to the bedrock ten to twenty feet downstream from the pipes. The department recommends monitor |
