2664644 Primary Facility 2017-11-22 Site Restoration No Violations Noted On November 22, 2017, the Department conducted a site restoration inspection at the Chesapeake Appalachia, LLC Polomski Location in Wyalusing Township, Bradford County. Andrew Hobbins represented the Department. Nobody was present for Chesapeake.

Generally the site was determined to meet the requirements of 25 Pa Code 102.22(a)(2)(I) Permanent Stabilization. Perennial vegetation was observed to be greater than 70% at the Polomski Location. No erosion or sedimentation issues were observed. The well pad has not been reduced in size, but PCSM has been addressed.

Two wells were in production during the inspection. The wells were labeled appropriately. Two production tanks and two GPUs were also on site. Puddles on the well pad surface were low in specific conductivity. No staining was observed on the pad.

Based on the conditions observed at the well pad, the Notice of Termination is recommended for approval.

2808180 Primary Facility 2018-11-28 Routine/Complete Inspection No Violations Noted Arrived onsite for a routine inspection at 12:02; the weather was ~30°F, windy, and snowing. There was a Chesapeake well tender onsite when I arrived but he left shortly thereafter.

The pad surface was snow and ice covered. The areas around the pad were also snow covered.

There were 2 properly tagged Marcellus wells staged for production. The well cellars were filled with water; no bubbling was observed. There were also 2 GPUs and 2 sand separators onsite and in secondary containment. The produced water tank containment had a few inches of ice inside it.

No obvious E&S issues were noted however the snow cover prevented a proper assessment of vegetative cover.
