2702423 Primary Facility 2018-03-05 Routine/Complete Inspection No Violations Noted On March 5, 2018, the Department conducted a routine inspection at the Chesapeake Appalachia LLC, Balduzzi Location in Wyalusing Township, Bradford County. The Department was represented by Andrew Hobbins. Nobody was present for Chesapeake.

The site sign, E & S plan and weekly inspection logs were on site and available for review. The site was well vegetated, and no erosion issues were observed. PCSM has not been addressed at the site and the well pad has not been reduced in size.

Two wells were in production during the inspection. The wells were both labeled and no bubbling was observed in the well cellars. Two production tanks and two GPUs were also on site. Puddles on the pad were low in specific conductivity.

No violations were observed during the inspection.

2822879 Primary Facility 2019-01-04 Routine/Complete Inspection No Violations Noted Arrived onsite for a routine inspection at 12:38; the weather was ~40°F and calm.

There were 2 properly tagged Marcellus wells staged for production. There were 3 well cellars onsite and filled with water; no bubbling was observed. There were 2 GPUs, produced water tanks, and sand separators onsite and in secondary containment.

The site was greater than 70% vegetative cover and appeared stable. The pad surface had been partially replace by vegetation mainly around the perimeter. It is recommended that any needed restoration activities or PCSM be addressed and an NOT submitted before the expiration of the ESCGP. The current permit appears to be valid until 7/6/20.

No violations were noted at the time of inspection.
