Inspection Data

Inspections Performed

Inspection ID Inspection Category Inspection Date Inspection Type Inspection Result Inspection Comments
2142806 Primary Facility 2013-01-14 Routine/Complete Inspection No Violations Noted 2 tanks, 2 treaters on location. Well in prod. Excessive water on location. No visible bubbling.
2166963 Primary Facility 2013-05-10 Routine/Complete Inspection No Violations Noted
2418203 Primary Facility 2015-10-21 Routine/Complete Inspection No Violations Noted Arrived 13:30, cloudy, warm windy. 3 cellars, two wells, 1 compressor, 2 sand separators and production units, 2 production tanks. 2H in production, cellar water-filled, no bubbles. 9x5: 14.008 psi, top gauge 309 psi, production casing 24.24 psi. 6H:locked and chained, water-filled cellar no bubbles. 9x5 14.54 psi, production casing: 2018 psi, tubing 34.01 psi. No violations were noted at this time. Left @ 13:45.
2565161 Primary Facility 2017-02-15 Routine/Complete Inspection No Violations Noted On February 15, 2017, at 1045 hrs. the department conducted a routine inspection at the Frisbee well pad location in Orwell Township, Bradford County. The department was represented by Andrew Hobbins, Water Quality Specialist. Nobody was present for Chesapeake.

There were two wells on site, both of which were in production phase. The wells were labeled appropriately and no bubbling was observed in the well cellar. The Department observed two production tanks and two GPUs on the well pad. A compressor unit was also observed on site. All puddles on the well pad were observed to be frozen. No conductivity readings were taken.

The well pad retains its original shape and has not been reduced in size. An earthen berm surrounds the well pad. A gap in the berm leads to a rock line ditch and then to an infiltration berm.

2755173 Primary Facility 2018-07-24 Routine/Complete Inspection No Violations Noted Warm/Cloudy at time of inspection
2772750 Primary Facility 2018-08-28 Site Restoration No Violations Noted NOT approval.

I took electrical conductivity readings of standing liquid on site with an Oakton PCSTestr 35. A small (~1/2" deep, 6" diameter) puddle on the pad returned a 1.0 mS/cm reading. JLP
