2586511 Primary Facility 2017-04-25 Routine/Complete Inspection No Violations Noted On April 20, 2017, at 1205 hrs., the Department conducted a routine inspection at the Chesapeake Appalachia LLC, Allen Location in Wysox Township, Bradford County. Andrew Hobbins, Water Quality Specialist, represented the Department. Nobody was present for Chesapeake.

The site sign, E&S plan and weekly inspection logs were on site and available for review. The slopes of the well pad were well vegetated. No erosion issues were observed. Two infiltration berms were installed and and well vegetated.

One well was in production during the inspection. The well was labeled and no bubbling was observed in the well cellar. Two production tanks and one GPU were also on site. The production tanks were on secondary containment. Liquid within the containment was low in specific conductivity. There was no staining observed on the well pad surface. Several large puddles were observed on the well pad surface. All of the puddles tested had specific conductivity readings less that 200 uS/cm.

2602099 Primary Facility 2017-06-06 Site Restoration No Violations Noted On June 6, 2017 at 1025 hrs., the Department conducted a site restoration inspection at the Chesapeake Appalachia, LLC (Chesapeake) Allen Location in Wysox Township, Bradford County. Andrew Hobbins, Water Quality Specialist, represented the Department. Nobody was present for Chesapeake. A Notice of Termination (NOT) was received by the Department on June 5, 2017.

Overall the site was determined to meet the 70% perennial vegetation requirement. The slopes of the well pad and the topsoil stockpile were well vegetated. All temporary E&S controls were observed to be removed from the site. The well pad has not been reduced in size, but PCSM has been addressed with the installation of two infiltration berms.

One well was in production phase during the inspection. One GPU and two production tanks were also on site. Puddles on the pad surface were low in specific conductivity. There was no staining observed on the well pad surface.

2850903 Primary Facility 2019-03-07 Routine/Complete Inspection No Violations Noted On March 07, 2019 Department representative Jared Jacobini conducted a routine site inspection at Chesapeake Appalachia LLC Allen 1H-6H pad. The site sign, drilling permits, monthly E&S inspection log, and emergency response plan were onsite for review.   The E&S inspection log was reviewed; the last routine inspection was conducted on 01/30/19. Infiltration berms (1&2) and rock lined channels are functioning properly. No erosion issues were observed. The site is stable and portions that were not snow covered were well vegetated meeting the 70% uniform, perennial, vegetative cover.

A total of one producing well was located on this well pad and tagged with API/well permit number. No bubbling within the cellars was observed as they contained frozen snow and ice. There is one GPU and two steel vertical production fluids tanks onsite. Production fluid tanks were stationed in secondary containment but the GPU was not. A Methanol injection unit was present and within containment.

No violations noted.
