Pressure Build-up Test (PBUT) being performed on the 5H and the 2H as part of an ongoing migration investigation. I was accompanied by Eric Rooney (DEP Geologist), we met with Joel Mincer (lease operator for Chesapeake) and Tom Messing of Leidos. The wells were shut in on 6/13/16 at 10:15a.m. It was noticed that the pop off valves were still on the (28x20), (20x13¿), and (13¿x9¿) vent pipes on the 2H and on the (28x20) and (20x13¿) vent pipes on the 5H.

Tom called Charlie Olmsted of Chesapeake to let him know of the pop off valves and Charlie said the PBUT would be rescheduled for sometime in July. No gas samples or pressure reading were taken. I left the site at 10:50a.m.

2502271 Primary Facility 2016-07-18 Routine/Complete Inspection Outstanding Violations - No Viols Req'd At time of inspection, arrived on site at 13:10p.m. for a post 72hr pressure build up test (PBUT), the cellar was empty, I met with lease operators Joel Mincer and Joel Failie. They were setting up the wells for the 72hr PBUT, Joel Mincer had to go to the shop to get some connections, he returned about 15:00p.m. The production line was shut in at 15:30p.m. and the other annuli’s were shut in and set up with a digital recorder at 5:26p.m. The vent pipes from the 28" and 20" caps were set up with a 15lb pop off valve and they had a hose going to a plastic tank.

I left the site at 5:30p.m.

2503856 Primary Facility 2016-07-21 Follow-up Inspection Outstanding Violations - No Viols Req'd At time of inspection, arrived on site at 16:10p.m. to conclude the 72hr. Pressure Build-up Test (PBUT) being performed on the 2H and the 5H as part of an ongoing migration investigation. The cellar had water at the bottom with no bubbling noticed. I met with Joel Mincer (lease operator for Chesapeake), Tom Messing and Shauna Erway of Leidos. The wells were shut in on 7/18/16 at 17:26,Tom used a Gas Explorer gas meter by Bascom Turner calibrated on 7/21/16 to check for gas show at all annuli’s, gas samples were taken if gas was detected. Pressures were taken from a 30psi pressure gauge for the 28" and 20" annuli’s, and from the SCADA digital pressure gauges on the wells for the rest of the annuli’s. The 28" and 20" annuli’s were set up with 15psi pop off valves and connected to poly tanks, there was no gas detected at the tanks. We started taking pressures at 17:25 on the 5H.

I left the site at 18:30p.m.

2575953 Primary Facility 2017-03-20 Routine/Complete Inspection No Violations Noted On March 20, 2017, the Department conducted a routine inspection at the Chesapeake Appalachia, LLC (Chesapeake) Rexford Location in Orwell Township, Bradford County. Andrew Hobbins, Water Quality Specialist, represented the Department. Nobody was present for Chesapeake.

The E&S plan, site sign and weekly inspection reports were on site and available for review. The site was mostly covered in about 8 inches of snow. PCSM has not been addressed at the site.

There were two well in production phase during the inspection. Both wells were labeled and no bubbling was observed in the well cellar. Two production tanks and two GPUs were on site. The production tanks were on secondary containment.

The ESGGP-2 permit expires 1/11/2021. A major modification was approved on January 12, 2016 to address PCSM. According to Department records, there are no active well permits for the Rexford Location. The well pad retains its original shape and has not been reduced in size. 2591650 Primary Facility 2017-05-08 Routine/Complete Inspection Outstanding Violations - Viols Req'd At time of inspection, arrived on site at 12:00p.m. for a pre-PBUT inspection.

The cellar was about three quarters full of water with no bubbling noticed. The (28x20) annulus was set up with a pop off valve and a 60psi pressure gauge with 0psi on it. The (20x13) annulus was set up with a digital recorder and a 60psi pressure gauge with 0psi on it. The (13x9) annulus was set up with a SCADA digital pressure gauge with 0psi on it and the valve going to the production tank was closed. The (9x5) annulus was set up with a SCADA digital pressure gauge with 12psi on it and the valve going to the production tank was closed. The 5½ production casing was open and venting to the production tank with 0psi on the SCADA pressure gauge. There was 858psi on the production tubing. I left the site at 13:05 p.m.

2593884 Primary Facility 2017-05-11 Follow-up Inspection Outstanding Violations - Viols Req'd At time of inspection, arrived on site at 8:00a.m. to conclude the 72hr. Pressure Build-up Test (PBUT) being performed on the 2H and the 5H as part of an ongoing migration investigation. The cellar was about half full of water with no bubbling noticed. I met with Joel Fairlie of Chesapeake, Tom Messing and Shauna Erway of Leidos. The wells were shut in on 5/8/17 at 8:15a.m. Tom used a Gas Explorer gas meter by Bascom Turner to check for gas show at all annuli’s, gas samples were taken if gas was detected. Pressures were taken from SSI Technology Inc. pressure gauge for the 28" and 20" annuli’s, and from the SCADA digital pressure gauges on the wells for the rest of the annuli’s. The wells were still producing.

We started taking pressures at 8:20a.m. on the 5H. I left the site at 9:15a.m.

2756116 Primary Facility 2018-07-11 Routine/Complete Inspection Outstanding Violations - Viols Req'd Remarks: At time of inspection. On site to check for gas presence, gas flows and take samples from the annuli of the 2H and 5H wells. I was accompanied by DEP OGI Supervisor, Mike Panettieri and DEP OGI, David Schoonover. We met with Chesapeake Reps. Max Goff (Operations Engineer), Lester Peterson (Optimization Technician), Joe Riley (Well Tender) and Leidos Rep. Tom Messing.

- Lester used an Eco meter to check the fluid level in the (9x5) and (5/Prod) annuli on 7/5/18. Tom used a Gas Explorer gas meter (calibrated on 7/10/18) to check for gas shows at annuli and I used an Altair 5X gas meter (calibrated on 7/9/18) to confirm the results. Joe used a Digital Manometer 621 with a 1/4" orifice to check for any flow rates at annuli. There were no measurable gas flow rates detected at any annuli, cemented to surface, of the 2H or 5H wells at time of inspection. Both wells (28"x 20"), (20"x 13 3/8") and (13 3/8"x 9 5/8") annuli had been open and venting for several weeks prior to this inspection. The results of
