2649459 Primary Facility 2017-10-13 Site Restoration No Violations Noted On October 13, 2017, the Department conducted a site restoration inspection at the Chesapeake Appalachia LLC, Cerca Location in Wyalusing Township, Bradford County. The Department was represented by Andrew Hobbins. Two lease operators were on site for Chesapeake. A Notice of Termination was received by the Department on September 20, 2017.

The site sign, E & S plan and weekly inspection logs were on site during the inspection. No erosion or sedimentation issues were observed and the site was well vegetated. Portions of the cut and fill slope were observed to be stabilized with rock. The well pad has not been reduced in size, but PCSM has been addressed at the site.

Two wells were in production during the inspection. Both wells were labeled appropriately. Two production tanks and two GPUs were also on site. No staining was observed on the well pad surface. Puddles on the pad tested low in specific conductivity.

No violations were observed. Based on the conditions observed during the inspec

2808191 Primary Facility 2018-11-28 Routine/Complete Inspection No Violations Noted Arrived onsite for a routine inspection at 12:47; the weather was ~30°F, breezy, and light snow. The pad surface was covered with ice and snow.

There were 2 properly tagged Marcellus wells staged for production. There were 2 GPUs and sand separators onsite and in secondary containment. The containment around the produced water tanks contain a few inches of ice.

Some taller vegetation was visible through the snow. A thorough assessment of vegetative cover was unable to be done. However, no obvious E&S issues were observed.

It is recommended that produced water tanks and other equipment be inspected and repaired or replaced as needed.

No violations noted.
