2769557 Primary Facility 2018-08-22 Incident- Response to Accident or Event No Violations Noted I arrived at the site at 11:45 in response to a spill of an estimated (43) bbls (barrels) of production fluid to secondary containment. The weather conditions are partly sunny skies and breezy with temps in the mid 70's. On 8/20/18 at 11:36 AM CHK submitted a Deficient Tank Inspection for unconventional wells per section 78a.57(i) Notification, for the Alderfer 2H well. The notice indicated the following information. The date and time of the tank inspection was 8/20/18 at 8:30 AM. The tank contents are produced water. The tank is (400) barrels in capacity. A small leak was observed in the sidewall of the tank resulting in an estimated (43) barrels of fluid lost. There were no impacts as a result of the spill and all fluid was contained in secondary containment. The corrective plan of action is to replace the tank and keep the wells shut in until the tank is replaced. I met with Bill Oliver and Jason Crawford, measurement techs of AMI Incorporated shortly after I arrived to the site. AMI informed me t
