2850741 Primary Facility 2019-03-08 Routine/Complete Inspection No Violations Noted On March 08, 2019 Department representative Jared Jacobini conducted a routine site inspection at Chesapeake Appalachia LLC BEEBE 1H-6H pad. The site sign, drilling permits, monthly E&Sinspection log, and emergency response plan were onsite for review. The E&Sinspection log was reviewed; the last routine inspection was conducted on 03/06/19. Containment berms have been along the pad border. The berms appeared to be functioning properly during the inspection. Infiltration trenches and rock lined channels are functioning properly. No erosion issues were observed. The site is stable and portions that were not snow covered were well vegetated meeting the 70% uniform, perennial, vegetative cover.

A total of one producing well was located on this well pad and tagged with API/well permit number. No bubbling within the cellar was observed as it contained frozen snow and ice.
