2586513 Primary Facility 2017-04-20 Routine/Complete Inspection No Violations Noted On April 20, 2017, at 1030 hrs., the Department conducted a routine inspection at the Chesapeake Appalachia, LLC (Chesapeake) Dewees Location in Rome Township, Bradford County. Andrew Hobbins, Water Quality Specialist, represented the Department. Nobody was present for Chesapeake.

The site sign, E&S plan and weekly inspection logs were on site and available for review. The slopes of the well pad were well vegetated. Areas on the fill slope were stabilized with rock. No erosion issues were observed. An infiltration basin was observed to be installed at the Dewees Location. The basin did not yet meet the 70% vegetation requirement.

One well was in production during the inspection. The well was labeled and no bubbling was observed in the well cellar. Two production tanks and one GPU were observed on site. The production tanks were on secondary containment. Conductivity readings were taken from the liquid in the containment structure which measured 3.5 mS/cm. The Department recommends removing the

Andrew Hobbins represented the Department. Nobody was present for Chesapeake.

The site sign, E & S plan and weekly inspection logs were on site and available for review. The last Chesapeake weekly inspection occurred on 1/19/2018. The site was well vegetated. No erosion issues were observed.

One well was in production during the inspection. The well was labelled appropriately. Bubbling was observed in the well cellar of the 2H well. Rich Herbstritt, OGI for the Department, was notified of the findings. Two production tanks were observed in secondary containment. The tanks had several corrosion spots on the exterior of the tank. The Department recommends the operator continue to monitor the tanks and replace them if necessary. One GPU was also observed on site. All puddles on the pad were observed to be frozen. No staining was observed on th

2689659 Primary Facility 2018-02-01 Follow-up Inspection Pending Cool/cloudy at time of inspection
observed bubbling from cellar close to well head.

Used a 5x multigas detector to check for gas as best as possible, was unable to get an accurate reading due to grating being placed over well cellar.

Unable to identify where exactly bubbling was comming from at time of inspection.

Called Eric Haskins (Regulatory operations manager Chesapeake), got no answer left a message for him to return my call.
